老年冠状动脉病变与血清脂蛋白相关磷脂酶A_2的关系

摘　　要：目的探讨血清脂蛋白相关磷脂酶A2(Lp-PLA2)水平与老年冠脉粥样硬化斑块稳定性的关系。方法 39例老年不稳定型心绞痛(UA)患者,28例稳定型心绞痛(SA)患者,血管内超声测定其"罪犯"病变的血管外弹力膜面积、管腔横截面积,斑块面积、斑块负荷,并利用虚拟组织成像技术对斑块成分进行比较;使用酶联免疫吸附法检测两组患者血清Lp-PLA2水平。结果 UA组和SA组患者的"罪犯"病变的管腔横截面积,斑块面积、斑块负荷差异均无统计学意义(P>0.05);与SA组比较,UA组斑块中脂肪组织、坏死组织所占比例增高,纤维组织比例降低,差异均有统计学意义(P<0.01),血清Lp-PLA2增高(P<0.01)。结论老年冠心病患者临床表现与血清Lp-PLA2关系密切。Objective To explore the relationship between lipoprotein - associated phospholipase A2 （Lp - PLA2 ） and coronan arteriosclerosis in elderly patiems. Methods Thirty - nine patients with unstable angina and twenty - nigh patients with stahle angina were enroiled in this study. The area of vascular external elastic membrane （EEM）, lumens area （LA）,plaque area（ PA ）, plaqne hurden （PB） were measured hy intravaseular ultrasound, The composition of plaque was analyzed by inlravascular ultrasound virtual hislology software. The serum level of Lp - PLA, was measured by ELISA . Results There were no significant differences of LA , PA, PB in＂ criminal＂ lesions hetween UA and SA groups （ P 〉 0.05 ）. Compared with SA group, the ratiou of adipose and necrotic tissue was increased,while the ration of fibrous tissue was decreased in UA group. The serum level of Lp - PleA, was increased. There was significant difference between the two groups（ P 〈 0.01 ）. Conclusion There is close relationship hetween clinical situation and serum level of l.p- PEA2 in elderly patients with coronary arteriosclerosis.
