外伤性脾破裂的螺旋CT诊断价值

摘　　要：目的:探讨螺旋CT在外伤性脾破裂诊断中的应用价值。方法:对70例经手术及临床证实的外伤性脾破裂患者的CT表现进行回顾性分析,其中20例平扫后增强扫描。结果:70例患者,平扫可确定脾破裂诊断的52例,可疑脾破裂诊断的10例,未见异常8例,诊断敏感性88.6%、特异性74.3%。20例行CT平扫后增强扫描,全部患者均能明确诊断为脾破裂(敏感性100%、特异性100%)。结论:CT增强扫描在外伤性脾破裂诊断中非常重要,能明显提高诊断准确率,为临床治疗方案的选择提供可靠依据。Objective：To evaluate the diagnostic value of Spiral CT Scans for Traumatic Splenic Rupture.Methods： The findings of CT scans of 70 cases with traumatic splenic ruture confirmed by operation were retrospectively analyzed.Among them,additional enhanced CT scan was done in 20 cases.Results：52 patients of splenic ruture were diagnosed by CT plain scan,10 patients were suspected with splenic ruture,and nothing abnormal were detected in 8 patients（sensitivity 85.3%,specificity67.6%）.Enhanced CT scan were administrated after CT plain scan in 20 cases,and all of the patients were diagnosed as splenic rupture（sensitivity 100%,specificity100%）.Conlusions： It is important for enhanced CT scan in diagnosing traumatic splenic rupture,and obviously improving the diagnostic accuracy.It provides reliable information for clinicans to select proper therapy.
