摘 要:目的对低高密度脂蛋白-胆固醇(HDL-C)血症患者药物干预前后血浆一氧化氮合酶(Nitric oxide synthase,NOS)和一氧化氮(Nitric oxide,NO)进行研究,探讨低HDL-C血症致动脉粥样硬化的可能机制。方法根据血脂水平和颈动脉内膜-中层厚度(IMT)等选取119例(男69例,女50例)低HDL-C血症患者作为低HDL-C血症组和78例(男43例,女35例)正常人群作为健康对照组,分别测定受检人群(包括低HDL-C血症患者治疗达标后)血浆NOS(用化学比色法测定)和NO(用硝酸酶还原法测定)。结果与健康对照组比较,低HDL-C血症组血浆NOS(P<0.05)和NO(P<0.05)均显著降低,男性及女性亚组亦显著低于相应健康对照组(P<0.05);低HDL-C血症组(79例)治疗达标(HDL-C>1.04mmol/L)后,血浆NOS(P=0.041)和NO(P=0.038)均显著升高(包括男性及女性亚组P<0.05),而与健康对照组干预前后相比,无统计学意义(P>0.05)。结论低HDL-C血症患者血浆NOS和NO水平显著低于健康对照组(包括男性和女性亚组)并且治疗后(血浆HDL-C水平达标)较治疗前显著升高而与健康对照组干预前后相比无明显改变;低HDL-C可能通过NOS/NO途径参与介导动脉粥样硬化发生、发展的病理演变过程。Objective To elucidate the possible mechanism of low high-density lipoprotein cholesterol(HDL-C) leading to the development of artery atherosclerosis by analysing plasma nitric oxide synthase(NOS) and nitric oxide(NO) of patients with low HDL-C and normal controls after and before intervention of drugs. Methods According to the carotid intima-media layer thickness and the plasma HDL-C level et al,we assigned 119 patients(M=69, F=50) with low HDL-C to the trial group and 78 healthy adults(M=43,F=35) to the control group, and measured their plasma concentration of NOS and NO(including after intervention). Results In patients with low HDL-C, the plasma concentrations of NOS(P0.05) and NO(P0.05) were significantly reduced compared with healthy controls; In the low HDL-C male or female subgroup, the plasma concentrations of NOS and NO were significantly reduced(P0.05) compared with the related healthy controls; After intervention,the plasma concentrations of NOS(P=0.041) and NO(P=0.038) were significantly increased, and were obviously unchanged compared with before and after intervation of healthy controls. Conclusions The plasma concentrations of NOS and NO were both significantly reduced in patients with low HDL-C(including male and female subgroups) compared to the healthy control groups and were significantly increased after beyond the target compared to them before treatment. In the lowered HDL state, it is possible that NOS/NO passway takes part in the patheological processs of artery atherosclerosis.
