卡托普利与尼群地平单独或联用随机双盲研究

摘　　要：该研究为随机双盲并行研究，５１４ 例高血压患者经二周安慰剂洗脱后，随机分入三组，卡托普利组１７１例，５０～７５ ｍ ｇ／ｄ，尼群地平组１７２例，２０～３０ ｍ ｇ／ｄ，卡＋ 尼组１７１ 例，平均卡５０ ｍ ｇ／ｄ＋ 尼２０ ｍ ｇ／ｄ。结果显示：各组在１ 周时即有降压作用，卡尼组降压作用持续而平稳，疗程结束时，卡尼组及尼组达标率分别为７０．６％ ，６５．２％ ，二者之间无显著性差异，但与卡组４８．４％ 比较Ｐ值均小于０．０１，卡组咳嗽发生率达３６．２％ ，尼组面红、失眠、下肢肿较多，卡尼小剂量联用后，尼组常见副作用明显减少，且联用组较卡组更能明显改善患者生活质量。In this random double blind study,after 2 week run in period,514 patients with blood pressure(BP)≥140/90 mmHg were divided into three goups:captopril group(CG, n =171),treated with captoril 50～75 mg/d;nitrendipine group (NG, n =172),treated with nitrendpine 20~30 mg/d;nitrendipine and captopril group (NCG, n =171),treated with nitrendipine 20 mg/d and captopril 50 mg/d.The results showed that in the first week,BP reduced in some degree in all three groups,and the antihypertensive effect of NCG was more permanent and steady than that of the other two groups.At the end of the sixth month,control rate of NCG,NG and CG was 70 6%,65 2% and 48 4%,respectively,and the difference between the control rates of NCG,NG and the control rae of CG was significant( P <0 01).Cough,the side effect,occurred in 36 2% patients who received captopril,while flush,insomnia and ankle edema occurred more often in those who received nitredipine,less adverse effects were observed in combination group,furthermore,the quality of life(QoL) in NCG was improved more greatly than CG,while nitrendipine had slightly negative influence on QoL.
