空心钉内固定联合股方肌蒂骨瓣移植配合中药治疗中青年囊内型股骨颈骨折临床研究  

Effects of Hollow-screw Internal Fixation Combined with Bone Flap with Quadratus Femoris Pedicle Transplantation cooperating with Chinese Herbs on the Treatment of Intra-capsular Femoral Neck Fracture in Young and Middle-aged Patients

摘　　要：目的:探讨空心钉内固定联合股方肌蒂骨瓣移植配合中药治疗中青年囊内型股骨颈骨折的疗效。方法:70例患者随机分为两组:空心钉内固定联合股方肌蒂骨瓣移植配合中药组(治疗组)38例和单纯空心钉内固定组(对照组)32例,比较治疗前后两组患者手术时间、术中出血量、复位质量(Garden对线指数)、骨折愈合率、股骨头坏死率、术后功能恢复程度(按Harris髋关节功能评分标准进行评分)及血液流变学变化的差异。结果:治疗组手术时间长,术中出血量多,其复位质量、骨折愈合率及髋关节功能优于对照组(P<0.05),股骨头坏死率低于对照组(P<0.05),血液流变学各项指标优于对照组。结论:空心钉内固定联合股方肌蒂骨瓣移植治疗中青年囊内型股骨颈骨折疗效优于单纯空心钉内固定,中药口服明显改善患者血液流变学。Objective：To observe the effects of hollow-screw internal fixation combined with bone flap with quadratus femoris pedicle transplantation accompanying by Traditional Chinese medicine Taohong Siwu Decoction on the treatment of intracapsular femoral neck fracture（IFCF） in young and middle-aged patients.Methods：Seventy young and middle-aged patients with IFCF were divided into two groups： 38 of them received hollow-screw internal fixation combining bone flap with quadratus femoris pedicle transplantation and traditional Chinese medicine（treatment group）,and other 32 were treated by hollow-screw internal fixation alone（contrast group）.The operation time,bleeding amount,quality of reduction（Garden alignment index）,rate of bone healing,rate of femoral head necrosis,Harris score of the hip joint function,and changes of blood rheology after 2 months were compared in both groups.Results：Compared with the contrast group,a longer operation duration,more bleeding amount in the operation,better reduction,higher rate of bone healing,higher Harris score of the hip joint function,and lower rate of femorel neck necrisis were recorded in the treatment group（P0.05）,as well as better improved blood rheology.Conclusion：For the intracapsular fracture of femoral neck in the young and middle-aged,combination operation may be better than simple operation.Chinese herbs may help to improve patients＇blood rheology after the surgery.
