眼科Ⅰ类切口手术围手术期预防使用抗菌药物调查分析与合理性评价

摘　　要：目的调查和评价医院眼科Ⅰ类切口手术患者围手术期预防应用抗菌药物的情况。方法随机抽取医院眼科2009年100份Ⅰ类切口手术患者病历,对围手术期应用抗菌药物的种类、给药时机、用药时间、切口愈合情况、住院药品费用等进行统计分析,并进行合理性评价。结果 100例手术患者全部在围手术期全身和局部预防应用抗菌药物,全身用药包括左氧氟沙星51例,占51.0%,头孢呋辛23例,占23.0%;阿奇霉素13例,占13.0%;克林霉素7例,占7.0%;仅术前30 min^2 h给药1次者61例,占61.0%,术前给药1次、术后给药<24 h者10例,占10.0%,术后给药>24 h者29例,占29.0%;术后用药最长为4 d,平均用药时间为2.3 d。结论眼科不合理用药主要表现在预防用药选择不当、术后应用抗菌药物时间偏长等方面;应加强抗菌药物干预,强化医师围手术期预防应用抗菌药物的合理化意识。OBJECTIVE To investigate and evaluate the perioperative prophylactic use of antibacterials in patients of Ⅰ types incisions in Ophthalmology.METHODS Totally 100 examples of medical record data of discharged patients with type Ⅰ randomly were checked during 2009.Statistics and analysis was made according to application rate of antibacterial drugs,administer occasion and time,wound healing and drug charge in hospitalization′s patients to evaluate the rationality of above indexes.RESULTS Of the 100 cases,100 received prophylactic antimicrobials by local and total body.The antimicrobials by total body are lavofloxacin lactate（51.0%）,cefuroxime sodium（23.0%）,azithromycin for injection（13.0%） and clindamycin phosphate（7.0%）.61% patients only used antibacterials between 30min and 2h before surgery,10.0% patients used antibacterials between 30 min and 2 h before surgery and once in 24 h after surgery.The average time is 2.3 days and the longest time is 4 days.CONCLUSIONS The non-rational use of drug is inappropriate selection and longtime use of antimicrobials after operation.The interference of perioperative prophylactic use of antibacterials should be reinforced and it is urgent to intensify physicians′ consciousness about rational perioperative use of antibacterials.
