不同剂量阿呋唑嗪治疗输尿管下段结石的临床研究

摘　　要：目的:探讨不同剂量阿呋唑嗪在输尿管下段结石治疗中的有效性和安全性。方法:随机将83例输尿管下段结石患者分为4组.A组为对照组.B组每天加用2.5 mg阿呋唑嗪,C组每天加用5 mg阿呋唑嗪,D组每天加用7.5 mg阿呋唑嗪,观察对比4组患者结石排出率、排石时间、肾绞痛发生率、哌替啶使用剂量及体位性低血压等不良反应。结果:加用阿呋唑嗪的患者排石率提高.肾绞痛发生率下降,差异有统计学意义(P<0.05);B组的排石率和肾绞痛发生率与C组和D组比较.差异有统计学意义(P<0.05);C组和D组比较,差异无统计学意义。加用阿呋唑嗪的患者排石时间缩短.哌替啶使用剂量减少.差异有统计学意义(P.<0.01);B组的排石时间和哌替啶使用剂量分别与C组和D组比较.差异有统计学意义(P<0.05);C组的排石时间和哌替啶使用剂量和D组比较,差异无统计学意义。结论:阿呋唑嗪可以促进输尿管下段结石的排出.每天使用5 mg或7.5 mg治疗输尿管下段结石安全、高效。Objective：To evaluale the efficacy and safety of different doses Alfuzosin hydrochloride in the treat- ment of lower ureteral stones. Methods： 83 patients with distal ureter stones were randomly divided into 4 groups, such as A group （control group）, B group （Alfuzosin, 2.5 mg everyday）, C group （Alfuzosin, 5 mg everyday） and D group （Alfuzosin, 7.5 mg everyday）. The stone expulsion rate, the mean stone expulsion time, the renal colic rate, the doses of Pethidine and the side effect of Alfuzosin were studied in 4 groups. Results：The stone expulsion rate increased and the renal colic rate decreased in patients used Alfuzosin. There had significant difference between 4 groups （ P 〈0.05）. The stone expulsion rate and the renal colic rate had significant difference between B group and C group, D group （ P〈0.05）. But there was no significant difference between C group and D group. The stone expulsion time and the doses of Pethidine decreased in patients used Alfuzosin. There had significant difference between 4 groups （ P 〈0. 01）. The stone expulsion time and the doses of Pethidine had significant difference between g group and C group, D group （ P 〈0.05）. But there was no significant difference between group 3 and group 4. Conclusions：Alfuzosin can increase the stone expelling rate. Using 5 mg or 7.5 mg Alfuzosin every day in treatment lower ureteral stones is safe and high effective.
