摘　　要：目的探讨新的四维左心室半自动定量工具(4DLVQ)评价正常人左心室容积和射血分数的可行性和可重复性。方法 采用新的实时三维超声心动图(RT3DE)心室容积分析工具4DLVQ和传统的RT3DE分析工具TomTec分别测量24名健康志愿者左心室舒张末容积(LVEDV)、左心室收缩末容积(LVESV)及左心室射血分数(LVEF),分析4DLVQ测量正常人左心室容积和射血分数的可行性和可重复性。结果 与TomTec比较,4DLVQ测定左心室容积和射血分数耗时明显减少(P<0.05),二者测得的LVEDV、LVESV及LVEF差异无统计学意义(P均>0.05)。采用两种分析工具测定LVEDV、LVESV及LVEF相关性良好(r=0.72、0.62、0.53,P均<0.05),Bland-Altman分析结果显示二者一致性高。与TomTec比较,采用4DLVQ测定LVEF的观察者内和观察者间变异系数小、组内相关系数(ICC)高。结论 4DLVQ分析速度快、操作简便、重复性好,是测定左心室容积和功能的一种新的分析工具。Objective To investigate the feasibility and reproducibility of the new 4-dimensional left ventricular semi-automated quantification tool（4DLVQ） in measurement of left ventricular volume and ejection fraction in normal adults.Methods Real time three-dimensional echocardiography（RT3DE） were performed on 24 healthy volunteers.In the apical 3D full-volumetric images,left ventricular end-diastolic volume（LVEDV）,left ventricular end-systolic volume（LVESV） and left ventricular ejection fraction（LVEF） were measured using 4DLVQ and TomTec,which was one of the standard RT3DE tool.The feasibility and repeatability of 4DLVQ were evaluated.Results The analysis time of 4DLVQ was significantly shorter than that of TomTec（P0.05）.There was no significant difference in LVEDV,LVESV and LVEF between the two methods（all P0.05）.LVEDV,LVESV and LVEF measured with 4DLVQ had good correlation with those measured with TomTec（r=0.72,0.62,0.53,all P0.05）.Bland-Altman analysis revealed good agreement in LVEDV,LVESV and LVEF between 4DLVQ and TomTec.Compared with TomTec,4DLVQ showed lower intraobserver and interobserver variabilities and higher intra-class correlation coefficient（ICC）.Conclusion 4DLVQ is a novel,simple,feasible and reproducible tool for measurement of left ventricular volume and ejection fraction.
