额部阶梯状皮瓣修复半鼻缺损的临床应用

摘　　要：目的探讨应用额部阶梯状皮瓣修复半鼻缺损的可行性。方法通过对额部相应区域血管的解剖学研究，设计额部阶梯状皮瓣，掀起后旋转至受区并折叠修复半鼻缺损。结果自2005年5月至2010年3月共修复先天性及后天性原因所致的半鼻缺损28例，术后皮瓣全部成活，外鼻两侧结构基本对称协调。电话随访1～3年，再造鼻翼形态良好，鼻腔通气良好，取得美学与功能的双重效果。结论额部阶梯状皮瓣是修复半鼻缺损的理想皮瓣。Objective To explore the feasibility of clinical application of the forehead ladder-shaped flap in repairing heminasal defect. Methods Though anatomical study of the blood vessels in forehead, the forehead ladder-shaped flap was designed, lifted and rotated backwards to the wound and then folded to repair heminasal defect. Results From May 2005 to March 2010, 28 cases of heminasal defect resulted from congenital and acquired reasons were repaired with the method mentioned above. The flaps survived totally, and the bilateral structures of the nose were symmetrical postoperatively. After 1 - 3 years following up by phone call, all cases were satisfied with the good reconstructed nose, good ventilation and both functional and aesthetic effects. Conclusion The forehead ladder-shaped flap is an ideal flap for repairing heminasal defect.
