摘  要:目的调查老年性关节损害中是否存有淀粉样变以及淀粉样变的类型。方法刚果红染色结合偏振光观察淀粉样变;免疫组织化学观察淀粉样变前蛋白类型;MALDI-TOF/MS结合DNA测序分析TTR蛋白基因是否发生突变。结果关节组织内发现刚果红染色阳性以及偏振光观察发出绿色荧光,确定淀粉样变沉积。免疫组织化学染色证实淀粉样变前蛋白为TTR蛋白。TTR基因和蛋白两方面同时检测结果,提示未发生基因突变。结论老年性关节损害患者关节组织内存有淀粉样变,淀粉样变前蛋白类型为TTR蛋白。Objective To investigate amyloidosis in age-related joint damage tissue and subtype of amyloid precursor proteins(APP).Methods Congo red staining and polarized light microscope were used to find amyloid in joint section from elderly patients who need for joint replacement surgery.Immunohistochemistry was to determine classification of APP.Combined with matrix assisted laser desorption ionisation time-of-flight mass spectrometry(MALDI-TOF/MS)analysis for serum transthyretin(TTR) and sequencing polymerase chain reaction(PCR),TTR gene mutation was analyzed.Results Congo red was found to be positive in joint tissue and green birefringence was observed under polarized microscope,indicating amyloidosis.With immunohistochemical staining,the amyloid protein was determined to be TTR which is wild type,as shown by both MALDI-TOF/MS analysis for protein molecular weight and PCR analysis for gene sequencing.Conclusions Amyloidosis is happened in joint tissues from patients with age-related joint damage and APP is wild type TTR.
