摘　　要：目的将伤残调整生命年(disability-adjusted life years,DALY)引入石林县健康评价领域,为石林县的疾病防治工作提供科学依据。方法抄录石林县2004年全死因调查资料,采用世界卫生组织推荐的方法计算。结果 (1)按死亡造成的疾病负担排序,早死损失寿命年(years of life lost,YLL)前5位疾病是呼吸系统疾病、意外伤害、循环系统疾病、蓄意伤害、恶性肿瘤;(2)无论何种排序,呼吸系统疾病都是石林县主要疾病负担。DALY指标排序精神神经疾病高居疾病负担原因首位;(3)不同性别、民族疾病负担有所不同。结论 (1)常规死因资料中的漏报、错报、死因不明等情况都会影响YLL的准确性,所以需要采用相应的方法对人口学数据进行适当调整;(2)重点加强边远地区的非传染性疾病的预防和干预。Objective The study aims to develop a method for evaluating BOD for residents in Shilin county using disability adjusted life year（DALY）,so as to provide scientific guide for rural dweller＇s diseases prevention and treatment.Methods The all-causes-of-death survey data in Shilin county in 2004 was collected and calculated by disability adjusted life year（DALY） approach which was recommended by the World Health Organization.Results（1）The five leading causes of YLL（in descending order）： Respiratory diseases,Unintentional injuries,Cardiovascular diseases,Intentional injuries,Malignant neoplasms;（2）No matter which sort,respiratory diseases was the leading disease.When sorted by DALY,neuro-psychiatric conditions was at the first place in all diseases;（3）Burden of disease was different in different race or gender.Conclusions（1）Underreporting,misinformation and dysoemia data in regular death information could influence the accuracy of YLL.Thus the corresponding method should be used to adjust demographic data and get the correct result;（2）Prevention and intervention on non-infectious chronic diseases should be emphasized on remote villages.
