HBV前S_2蛋白检测与HBV-DNA和乙肝二对半模式相关性研究

摘　　要：目的探讨HBV前S2蛋白检测与HBV-DNA和乙肝二对半标志物模式的相关性及其临床价值。方法应用聚合酶链反应法和酶联免疫吸附实验法对216例HBV-DNA和HBV血清标志物、前S2蛋白同时进行平行检测。结果 "大三阳"患者前S2蛋白和HBV-DNA阳性率分别为42.9%和86.7%,"小三阳"患者前S2蛋白和HBV-DNA阳性率分别为3.4%和13.6%,血清前S2蛋白和HBV-DNA检出率比较,二者密切相关(χ2=38.01,P<0.005);以HBV-DNA检测为标准,HBeAg的敏感度为87.7%,前S2蛋白的敏感度为43.4%,HBeAg和前S2蛋白与HBV-DNA的总符合率分别为87.5%和68.9%。结论 HBV前S2蛋白检测对HBV感染的诊断价值较低,但可作为HBeAg和HBV-DNA的补充指标。Objective To investigate the clinical significance and relationship among Pre-S2, common types of HBV serum immune markers and HBV-DNA. Methods Using polymerase chain reaction to detect HBV-DNA and enzyme linked immunosorbent assay technique to detect Pre-S2 ,common types of HBV-DNA serum immune markers in the meantime. Results Pre-S2 protein and HBV-DNA positive rates of patients with positive HBsAg,HBeAg,HBcAb markers was 42.9% and 86.7% respectively; but Pre-S2 protein and HBV- DNA positive rates of patients with positive HBsAg,HBeAg,HBcAb markers was 3.4% and 13.6% respectively;The positive rates were highly correlation between Pre-S2 protein and HBV-DNA in 216 cases （Х2 = 38.01, P 〈 0. 005 ） ; Taking positive HBV-DNA as the diagnostic criterium, sensitivity of Pre-S2 protein was 43.4%, sensitivity of HBeAg was 87.7%, the over all coincidence of HBeAg and Pre-S2 protein to HBV-DNA was 87.5 % and 68.9% respectively. Conclusion The diagnostic value of Pre-S2 protein determination is low in the HBV cases. But, to some extent, the detection of Pre-S2 protein might complement HBe system and HBV-DNA clinical routine.
