腹部手术后早期肠内营养联合应用生长抑素的临床疗效观察

摘　　要：目的：探讨腹部各种手术后早期肠内营养支持（Early Enteral Nutrition,EEN）,与生长抑素（Somatostatin,SS）联合应用的效果与可行性。方法：将50例接受各种腹部择期手术的住院病人随机分为两组（n=25例）,两组均于手术后6h起始予相同方式的早期肠内营养支持,连续5~7d,实验组同时应用生长抑素,对照组未予。比较两组病人手术后48h腹痛腹胀的发生率、胃肠减压量以及手术后2d、7d肝肾功能及血糖、电解质等指标。结果：与对照组比较,实验组腹痛腹胀发生率（P=0.031）及胃肠减压量（P=0.002）降低,差异有统计学显著性。手术后2d、7d,两组间肝肾功能、血糖和电解质结果（P〉0.05）比较差异无统计学显著性。结论：各种腹部手术后早期肠内营养支持与生长抑素联合应用是安全有效、切实可行的围手术期治疗措施。Objective： To investigate the therapeutic effect safety and the resumption of gastro-intestinal function lf early enteral nutrition（EEN）combined with somastostatin（SS）in patients after abdominal surgery.Methods： 50 patients who received large or medium abdominal operations were randomized into groups including 25 cases of treatment group with SS on the basis of EEN and 25 cases of control group with EEN.The promotion of clinical manifestations and signs,gastrointestinal decompression,liver and renal function,blood glucose,electrolytes and so on were compared between the two groups.Results： As for Clinical manifestation and sign promotion,there was statistical significance in the alleviation of pain and distension of the abdominal after 48h（P=0.031） and the average volume of gastrointestinal decompression（P=0.002）between the treatment group and the control group.It indicated that the treatment group was better than the control group in therapeutic results.As for liver and renal function,blood glucose,electrolytes,there were no statistical significance between the treatment group and the control group（P0.05）,it indicating that the therapeutic effect of SS does not depress the effect of EEN.Conclusion： EEN combined with SS is a safe and effective peri-operative treatment and it can obviously alleviate clinical symptoms and increase the achievement ratio of expected treatment.
