活血化瘀药联合长球囊血管成形术治疗糖尿病足的疗效评价

摘　　要：【目的】观察和评估以中医活血化瘀法联合长球囊血管成形术治疗糖尿病足肢体缺血的可行性,并初步评估其疗效。【方法】选取65例(患病肢体共计83条)分为观察组和对照组2组。观察组35例给予常规内科治疗措施、长球囊血管成形术介入治疗联合活血化瘀通络中药(丹参注射液)治疗,对照组30例只给予常规内科治疗措施和长球囊血管成形术介入治疗。观察治疗前后患者的临床症状、血流动力学改变、溃疡愈合时间和肢体功能变化情况。【结果】介入手术成功率为91.5%。观察组糖尿病足患者截肢率为5.3%,治愈率为42.1%,总有效率为86.8%;对照组截肢率为13.9%,治愈率为16.7%,总有效率为58.3%,2组比较差异有显著性意义(P<0.05)。且治疗后2组的临床症状、溃疡愈合时间、肢体功能恢复情况、相关实验室指标比较差异均有显著性意义(P<0.05)。【结论】活血化瘀中药联合长球囊血管成形术是治疗糖尿病足的一种有效手段,能改善下肢缺血引起的症状,减少截肢率,且未见严重的并发症和毒副反应。Objective To evaluate the therapeutic effect of blood-activating and stasis-resolving Chinese herbal medicine combined with long balloon angioplasty for the treatment of diabetic foot. Methods Sixty-five patients with 83 diabetic feet were randomized into two groups. The treatment group （ N = 35 ） was given routine internal medical treatment, interventional therapy of long balloon angioplasty, and blood-activating and stasis-resolving Chinese herbal medicine Danshen Injection. The control group （N = 30） was given routine internal medical treatment and interventional therapy of long balloon angioplasty. The clinical symptoms, hemodynamie parameters, ulcer healing time, and diabetic foot function were observed before and after treatment. Results The successful rate for long balloon angioplasty was 91.5%. In the treatment group, the amputation rate was 5.3%, the cure rate was 42. 1% , and the total effective rate was 86. 8%. In the control group, the amputation rate was 13.9%, the cure rate was 16. 7% , and the total effective rate was 58.3%. The differences between the two groups were significant （p 〈 0.05 ）. The improvement of clinical symptoms, ulcer healing time, diabetic foot function, and hemodynamic parameters in the treatment group was superior to that in the control group （P 〈 0. 05）. Conclusion Blood-activating and stasis-resolving Chinese herbal medicine combined with long balloon angioplasty is effective for the treatment of diabetic foot, which can relieve the symptoms of lower limb ischemia, reduce amputation rate, without serious toxic or side effects
