红景天对先天性心脏病患者体外循环术后炎性因子表达的影响

摘　　要：目的通过研究红景天对体外循环(CPB)前、中、后血浆肿瘤坏死因子-α(TNF-α)、可溶性胞间黏附分子-1(sICAM-1)和可溶性血管细胞黏附分子-1(sVCAM-1)的变化,评估红景天对体外循环术后炎性反应的预防与治疗价值。方法将659例先天性心脏病体外循环心内直视手术患者按双盲法随机分为2组,红景天未干预组(CN)312例,红景天干预组(CI)347例。干预组于术前1 d、术中、术后7 d连续予以红景天1 mg.kg-1.d-1静脉滴注,其余治疗方案2组大致相同。酶联免疫吸附测定(ELISA)法测定术前1 d,CPB开始后30 min,停CPB后2h,术后第1天、第3天、第5天血浆TNF-α、sICAM-1和sVCAM-1浓度。结果 2组患者在CPB开始后30 min及术后血浆TNF-α、sICAM-1和sVCAM-1浓度较术前显著升高,差异有统计学意义(P<0.01);CBP开始后及术后干预组与未干预组比较,未干预组TNF-α、sICAM-1和sVCAM-1浓度升高明显,二者差异有统计学意义(P<0.01);TNF-α与sICAM-1、sVCAM-1之间呈正相关(r=0.671,0.698,P<0.01)。结论红景天对TNF-α与sICAM-1、sVCAM-1等炎性因子有显著调节抑制作用,对预防与治疗CBP下心脏手术患者术后炎性反应有很好的临床实用价值。Objective To investigate the effects of the integripetal rhodiola herb on TNF-α,sICAM-1 and sVCAM-1 in serum before,during and after undergoing cardiopulmonary bypass（CPB） and on inflammatory reaction induced by CPB.To evaluate the clinical value of the integripetal rhodiola herb in prevention and treatment of postoperative inflammatory response in patients with congenital heart disease induced by CPB.Methods Six hundred and fixty-nine cases with congenital heart disease undergoing open heart surgery with CPB were randomly divided into the integripetal rhodiola herb intervention group（n=347） and non-intervention group（n=312） by double blind method.The intervention group received integripetal rhodiola herb 1 mg·kg^-1·d^-1 iv gtt from one day prior to operation to 7 day after operation in addition to the therapy for the non-intervention group,while the non-intervention group received no integripetal rhodiola herb,the remaining treatment programs of two groups are of the same.The serum levels of TNF-α,sICAM-1 and sVCAM-1 were determined by enzyme-linked immunosorbent assay,one day prior to surgery,15 minutes since the start of CPB,1 hour after CPB and at 1,3,5 day after operation,respectively.Results The serum levels of TNF-α,sICAM-1 and sVCAM-1 during and after CPB surgery were significantly higher than those before surgery（P〈0.01）;In non-intervention group,the serum levels of TNF-α,sICAM-1 and sVCAM-1 during and after CPB surgery were significantly higher than those in the intervention group（P〈0.01）.After operation,the intervention group showed an earlier recovery than in the non-intervention group.At the same time,the serum levels of TNF-α level correlated positively with sICAM-1 and sVCAM-1 level（r=0.671,P〈0.01）.Conclusion The integripetal rhodiola herb could effectively regulate and inhibit effects on inflammatory factor：TNF-α,sICAM-1 and sVCAM-1,it has great clinical value in the prevention and treatment of postoperative inflammatory reactions in patients with congenital
