股骨髁间粉碎骨折的手术治疗及疗效观察

摘　　要：目的观察手术切开复位内固定治疗股骨髁间严重粉碎性骨折的临床疗效。方法股骨髁间严重粉碎性骨折按Neer分型为型,AO分型为C2或C3型,本组62例患者均采用手术切开复位内固定治疗。结果 62患者中60例获得随访,时间10～30个月（平均18个月）。58例骨折一期愈合,2例给予二期植骨后骨折愈合。骨折愈合时间3～18个月,平均6个月,未出现钢板螺钉松动移位、骨折塌陷、复位丢失等。术后膝关节功能按Kolmert标准评定,优19例,良33例,可5例,差3例,优良率为86.67%。结论对于股骨远端粉碎性骨折,通过关节内骨折的复位、坚强内固定的应用以及早期有效的功能锻炼,均能获得良好的治疗效果。Objective To observe the clinical effects of open reduction and internal fixation of severely comminuted intercondylar fracture.Methods According to Neer classification is Ⅳ,According to AO classification is C2 or C3,the 62 cases of severely comminuted intercondylar fracture who had been treated with open reduction and internal fixation.Results 60 patients were followed up for a period averaged 18months（10 to 30months）.The results were excellent.Their functional recovery was evaluated according to Kolmert.19 cases were rated as excellent,33 as good,5 as fair and 3 as poor.Conclusion Open reduction and internal fixation is a good method of treatment for severely comminuted intercondylar fracture.
