双源CT冠状动脉CTA不良反应的分析与处理

摘　　要：目的:分析双源CT冠状动脉CTA检查过程中出现的各种不良反应的发生原因与处理。方法:收集2010年1月1日至2010年5月18日间1 405名接受冠状动脉CTA病例,结合过敏史及检查前的处理,分析不良反应的发生率以及相应的处理。结果:检查者中既往有药物过敏史或过敏性疾病患者79人,发生不同程度不良反应者18人;无过敏史者1326人,出现不良反应者35人(P<0.01)。有过敏史受检者42位检查前接受地塞米松处理,37位未接受地塞米松处理,均有9人发生不同程度不良反应(P>0.05),但未接受地塞米松处理的患者中有2例发生过敏性休克。53例发生不良反应受检者经处理均好转。结论:双源CT冠状动脉CTA发生不良反应时,应根据不同程度采取方法及时处理,受检者有无药物过敏史等高危因素与冠状动脉CTA不良反应的发生有明显相关,检查前用地塞米松预处理不能明显降低冠状动脉CTA不良反应的发生,但可能减轻不良反应的程度。Objective： To analyze the incidence rate and and management of adverse reaction in dual-source CT coronary arteri-ography（DSCT-CCTA）.Methods： Total 1405 patients who received DSCT coronary angiography examination were enrolled in this study from Jan 1,2010 to May 18,2010.Considering both the allergic history and the pre-examination management,the incidence rate and the medical managements of adverse reaction were recorded and analyzed.Results： There were 79 patients who had a histo-ry of drug allergy or other allergic diseases.Adverse reaction occurred in 18 patients of those patients.The other 1 326 patients had no history of allergic disease,and adverse reaction occurred in 35 patients（P 〈 0.01）.Among those patients of allergic history,42 cases received pre-examination treatment of Dexamethasone.There was no significant difference of the incidence rate of adverse reac-tion between those patients with or without pre-examination treatment（P 〉 0.05）.However,allergic shock occurred in two patients without pre-examination treatment of Dexamethasone.Those 53 patients with adverse reaction recovered after proper medical treat-ment.Conclusion：When adverse reaction occurs during the DSCT-CCTA,patients should be treated individually according to the degree of adverse reaction.Adverse reaction was highly correlated to allergic history.Pre-examination treatment of Dexamethasone could not reduce the incidence rate of adverse reaction significantly,but may relieve the degree of adverse reaction.
