聚-L-乳酸/β-磷酸三钙可吸收内固定器对犬胫骨骨折愈合的影响

摘　　要：目的研究聚-L-乳酸/β-磷酸三钙(PLLA/β-TCP)可吸收接骨板及螺钉对犬胫骨横行骨折的固定效果。方法 10只家犬制成双侧胫骨中段横行骨折模型,右侧用PLLA/β-TCP可吸收接骨板及螺钉固定作为实验组,左侧接骨钛板及螺钉固定作为对照组,术后4、8、12周进行大体、X线、扫描电镜及材料弯曲强度测试。结果大体观察:各观察时间点,实验组、对照组愈合过程无明显差异,术后12周,实验组、对照组均已骨性愈合,愈合过程中可见PLLA/β-TCP材料吸收征象。X线:实验组、对照组骨折对位对线好,未发生骨折移位,均产生大量骨痂并获得良好愈合。SEM观察:原PLLA/β-TCP材料致密度高,基本没有空隙;术后4周出现了少量的小孔;术后8周出现了大量被侵蚀的孔洞;术后12周材料内部孔洞进一步侵蚀,表面形成壳层结构。弯曲强度测试:原PLLA/β-TCP材料弯曲强度为148MPa;术后4周143MPa;术后8周130MPa;术后12周103MPa。结论 PLLA/β-TCP可吸收接骨板和螺钉具备足够的机械强度以维持胫骨横行骨折断端间的稳定,能维持胫骨骨折愈合过程的完成。Objective To study the effect of PLLA/β-TCP absorbable plate and nail by internal fixation on fracture healing of dog tibia.Methods A total of 10 dogs were used to prepare bilateral middle tibia fracture models.Right tibia were treated with internal fixation of PLLA/β-TCP plates as the experimental group,and the left tibia were fixed with titanium plates as control.Gross observation,radiography,SEM observation and mechanical property of materials were performed at 4,8,12 weeks after operation.Results Gross observation：at each check point,there were no difference between experimental group and control group.At 12 weeks after operation,both sides became into bone union completely,but there were absorbed sign of PLLA/β-TCP during observation.X-ray examination：both the experimental group and control group were fixed solidly and produced generous callus,and the fractures healed well in both sides.SEM observation：PLLA/β-TCP materials had a high density,and no pores were found in it.There were erosive pores during the observation after operation.Mechanical property：the primary bending strength of PLLA/β-TCP was 148MPa,after 4,8,12 weeks degradation in vivo,the bending strength of PLLA/β-TCP remains 143 MPa,130 MPa and 103 MPa,respectively.Conclusion PLLA/β-TCP plates and nails were provided with enough mechanical strength to maintain the stability of the tibia fractures during the healing process.
