培土生金法对慢性阻塞性肺疾病稳定期生存质量与营养状态的影响

摘　　要：目的:探讨培土生金法对慢性阻塞性肺疾病(COPD)稳定期患者生存质量与营养状态的影响。方法:将60例COPD稳定期患者随机分为两组,治疗组30例给予培土生金中药治疗,并配合吸入噻托溴胺(思力华)治疗,对照组30例单纯采用吸入思力华治疗。观察两组治疗前后肺功能变化情况,比较中医症状体征、生存质量评分及营养指标的变化。结果:用药6个月后,两组肺功能均有所改善;症状体征变化比较,治疗组临床控制2例,显效9例,有效15例,无效2例,总有效率92.86%;对照组临床控制1例,显效3例,有效17例,无效5例,总有效率80.77%。治疗组优于对照组(P<0.05);两组治疗后的生存质量评分均较治疗前有改善(P<0.05),且治疗组改善更加明显,优于对照组(P<0.05);两组治疗后血清白蛋白(ALB)、总蛋白(TP)均较前有改善,且治疗组优于对照组(P<0.05),治疗组的实际体重占标准体重百分比与本组治疗前及治疗组治疗后相比均有统计学意义(P<0.05)。结论:培土生金法配合吸入思力华可更好地改善慢阻肺患者稳定期的生存质量与营养状态。Objective： To investigate the effect of method of strengthening earth to generate metal combined with inhaling Si-li-hua on living quality and nutritional status among the cases suffered from chronic obstructive pulmonary in stable phase. Method： 60 cases with chronic obstructive pulmonary in stable phase were divided into two groups randomly. 30 cases in treatment group were treated with method of strengthening earth to generate metal combined with inhaling Si-li-hua, and the ones in control group were done with inhaling Si-li-hua. Before and after treatment, lung function, symptoms, signs, living quality scores and nutritional status were observed and contrasted. Result： After 6 months＇ treatment the lung functions were improved in the two groups. About symptoms and signs, in treatment group 2 cases were recovered, 9 ones excellent, 15 ones effective and 2 ones ineffective. The total effective rate was 92.86%. And in control group 1 case was recovered, 3 ones excellent, 17 ones effective and 5 ones ineffective. The total effective rate was 80. 77%. Treatment group did better than control group （ P 〈 0. 05 ）. After treatment, living quality scores in the two groups were all improved, but treatment group did better than control group （P 〈 0.05 ）. And After treatment, ALB and TP were increased, and also treatment group did better than control group （P 〈 0.05 ）. IBM% showed significant difference between before and after treatment in treatment group （ P 〈 0. 05 ）. Conclusion ： Method of strengthening earth to generate metal combined with inhaling Si-li-hua can improve living quality and nutritional status among the cases suffered from chronic obstructive pulmonary in stable phase.
