激素联合非甾体抗炎药滴眼治疗LASEK术后Haze的临床观察

摘　　要：目的观察准分子激光角膜上皮瓣下磨镶术(LASEK)术后单用妥布霉素地塞米松滴眼液(典必殊)或典必殊减量使用加普拉洛芬滴眼液(普南扑灵)对术后角膜上皮下雾状混浊(Haze)的疗效。方法采用随机对照方法,将LASEK术后出现2～3级Haze的患者90例(180只眼)均分为两组,试验组予典必殊与普南扑灵滴眼,对照组则单用典必殊滴眼,用药3个月,分别于术后不同时间观察Haze、眼压、裸眼视力。结果在术后2周和1个月时,对照组的眼压比术前有所增加,而试验组则轻微降低,且两组眼压有显著性差异(P<0.05);试验组术后电子验光视力比对照组明显提高且有显著性差异;对照组患者Haze稍有变化,而试验组则明显减轻或消失。结论激素联合非甾体类抗炎药使用对LASEK术后Haze安全有效,值得临床推广。Objective To compare the effects of pure Tobramycin Dexamethasone Eye Drops and dose-reducing Tobramycin Dexamethasone Eye Drops plus Pranoprofen Eye Drops for treating haze after LASEK implantation.Methods With randomized controlled method,90 patients（180 eyes） with 2-3 grade Haze after LASEK were selected and evenly divided into two groups.The trial group was treated with Tobramycin Dexamethasone Eye Drops and dose-reducing Tobramycin Dexamethasone Eye Drops plus Pranoprofen Eye Drops and the control group with Tobramycin Dexamethasone Eye Drops for 3 months.Haze,naked vision and intraocular pressure were observed at the different time after LASEK between the two groups.Results In two weeks and one month after operation,the intraocular pressure in the control group was increased than before operation,while which in the trial group was slightly decreased with significant difference（P 0.05）.In 2 weeks,1 month and 3 months,the electronic measurement of refractive optometry in the trial group was increased more than that in the control group with significant difference.By random checking in 2 weeks,1 month and 3 months,haze in the control group had slight improvement,but which in the trial group was obviously alleviated or disappeared and the vision was remarkably increased（P 0.05）.But no statistical difference existed（P 0.05）.Conclusion The combined use of hormone and non-steroidal anti-inflammatory drug（NSAID） is safe and effective to treat haze after LASEK,which is worthy of being popularized in clinic.
