摘　　要：目的探讨心脏远程监护系统的临床应用价值。方法 1315例用户均随身佩带心脏远程监护仪iHolter,采用CM1、CM5双通道记录和自动报警、无声模式,并实时监控。用户摘机后,均将预警器进行USB回放,并出具报告。结果 1 181例用于疾病诊断用户中,发现心电异常977例,均予以明确诊断;通过自动报警记录,发现恶性心律失常或明显ST-T改变等心电异常69例,均及时电话通知患者本人或家属,或采取相应预防措施,或及时救治;87例手术监护患者中,对4例心电异常者,接受自动报警发送的心电图后,即刻建议暂缓手术;26例起搏器患者中,因心慌发图显示恶性心律失常5例,因胸闷、憋气发图显示ST-T改变者2例,因头晕发图显示感知功能障碍者1例。对后者所安装的起搏器,及时将参数进行调整;47例保健干部中,自动报警记录见心电异常者4例,经相应处理后迅速缓解。结论心脏远程监护系统在疾病诊断、手术监护、起搏器随访和干部保健等方面具有较高的临床应用价值。Objective To study clinical significance of cardiac remote monitoring system. Methods 1 315 users were girded on cardiac remote monitoring apparatus using the mode of automatically and quietly warning, and monitor on reahime. After the apparatus was picked off, the electrocardiogram date storing in it were imported into computer through Universal Serial Bus and analyzed retrospectively. Results In 1 181 users for diagnosis ,977 cases occurring electrocardiogram abnormity were discovered and diagnosed definitely. In the records of electrocardiogram by automatically warning,69 cases of malignant arrhythmia and obvious ST-T change were discovered, and they or their family numbers were informed immediately, or were taken relevant preventive, or cure in time. In 87 users for surgical wardship ,4 cases of exceptional cardiogram were discovered through the records of electrncardiogram by automatically warning and the advice was given immediately to pause the operation. In 26 cases of pacemaker followup ,5 cases of malignant arrhythmia because of palpitation, 2 cases of ST-T change because of chest stuffiness or suffocation and 1 case of apperceive holdback because of dizziness were discovered. For the latter, some parameters of his pacemaker were modulated in time. In 47 users for cadre health care ,4 cases of exceptional cardiogram was discovered through the record of elec- trocardiogram by automatically warning and corresponding measure was brought into effect urgently. Conclusion It had a better applied significance using cardiac remote monitoring apparatus in diagnosis, surgical wardship, pacemaker followup, cadre health care, and so on.
