摘　　要：[目的]探讨出院延续护理在慢性伤口病人中的应用。[方法]对27例慢性伤口未愈合病人采取定时到医院复查或由家属带伤口照片等资料到医院复诊,由造口治疗师指导护理的方法实施延续护理。[结果]病人伤口顺利愈合24例,伤口缩小、变浅后失访2例,未愈合1例。[结论]对出院的慢性伤口病人实施延续护理有利于伤口的愈合。Objective：To probe into application of stretch nursing care in patients with chronic wound.Methods： A total of 27 patients with chronic wound did not healed were asked tocome back to hospital regularly to accept recheck up with family photos and other information.Then all cases were carried out stretch nursing care guided by stoma therapist.Results： All 24 patients with chronic wounds have healed smoothly,the wound reduced.However,wound of 2 cases became shallow.One case was not healed.Conclusion： To carry out stretch nursing for discharged patients with chronic wound is helpful for wound healing.
