摘 要:目的:探讨柴胡皂甙-d(SSd)治疗抗肾小球基膜型(GBM)肾炎的疗效及其机制。方法:采用抗GBM肾炎大鼠模型,用SSd在疾病早期和晚期两个阶段给药,观察SSd对其的疗效。结果:SSd治疗可减轻尿蛋白,降低血脂和减轻肾小球病理变化。同时,血清促肾上腺皮质激素(ACTH)、皮质酮、肾组织糖皮质激素受体(GR)、活性氧(RO)清除剂活性和红细胞免疫功能均增强。其中早期治疗效果尤为显著。结论:SSd对抗GBM肾炎大鼠有良好的治疗作用。其机制可能系SSd通过下丘脑-垂体-肾上腺(HPA)轴促进内源性糖皮质激素(GC)分泌,从而增强GC-GR系统的抗炎和免疫抑制作用。另外也与提高红细胞免疫、RO清除剂活性及改善异常脂质代谢等有关。Objective: To investigate the therapeutical effect of saikosapoind(SSd) on antiglomerulus basement membrane(GBM) nephritis in rat. Methods: Therapeutical effect of SSd on antiGBM nephritis in rat was observed in the heterologous and autologous phases. Results: SSd markedly reduced urine protein and blood lipid level, raised serum ACTH and corticosterone level. Meanwhile,immunofunction of RBC, renal glucocorticoid receptor(GR) and activity of reactive oxygen scavenger increased in rats with antiGBM nephritis, which were more significant in the heterologous phase. Conclusion: SSd has a good therapeutical effect on antiGBM nephritis in rat.The mechanism might be the enhanced antiinflammatory action and immunosuppression of GCGR system by improving HPA axis.Moreover,the elevated RBC immunofunction, RO scanvenger activity and ameliorated abnormal lipid metabolism might be related.
