摘 要:目的探讨严重冻伤后早期手术切痂同种异体皮移植修复重度冻伤创面的疗效。方法选取2000年3月~2010年3月在我院住院治疗的重度冻伤72例,根据治疗方法的不同,将常规治疗36例设为对照组,早期手术切痂同种异体皮移植36例为实验组,比较两组患者创面肉芽组织成熟程度、截肢(指、趾)率的情况。结果实验组肉芽组织成熟程度和截肢(指、趾)率均优于对照组。结论早期手术联合同种异体皮移植可防止重度冻伤创面渐进性坏死,减少肢体严重感染和坏疽发生率,最大限度地保留肢体长度和功能。Objective To investigate the therapeutic effects of early excision of eschar and allogenic skin grafting on the repair of severe frostbite wound surface.Methods 72 cases of severe frostbite hospitalized in our department were enrolled from March 2000 to March 2010.The patients receiving normal surgical treatment were served as control group(n=36),and those receiving early excision of eschar and allogenic skin grafting were served as grafting group(n=36).The mature degree of granulation tissue and extremity/toe/finger amputation rate were compared between the two groups.Results The mature degree of granulation tissue and extremity/toe/ finger amputation rate in grafting group were significantly better than those in the control group.Conclusion Early surgical operation combined with allogenic skin grafting can not only prevent severe frostbite wound surface from progressive necrosis,but also reduce severe extremity infection and gangrene as well as maximize the reservation of extremity length and function.
