中西药联合治疗9例肝囊性棘球蚴病和1例肺棘球蚴病及其B超图像分析

摘　　要：棘球蚴病足一种常见的寄生虫病，主要分为囊性棘球蚴病（cysticechinococcosis，CE）和泡性棘球蚴病（alveolar echinococcosis，AE），一直沿用外科治疗，但常引起复发，或因手术时囊液外漏，致使包虫扩散种植。近几十年国内外兴起的化学治疗以甲苯咪唑和苯硫眯唑为有效的抗包虫病药物。基于前者肠道吸收较后者差，现几乎为后者所取代。笔者曾研制消包胶囊（xiao-bao capsule），由槟榔、榧子、蛇脱、土鳖虫、露蜂房和穿山甲等6味药组成，每丸含药粉250mg。消包胶囊在小鼠实验或临床试用中均有较好的疗效，而且毒性低微，无不良反应。近年来，笔者采用中西药联合治疗CE患者，并在治疗前后作B超对比。该文总结了9例肝CE及1例肺CE患者疗效观察，治疗后B超图像显示，包虫囊肿由混合性回声团取代，开始显非均质性，以后变为均质性强回声团，并有多发性斑状钙化形成，达到治愈的目的．Eehinoeoccosis is a common parasitic disease, including cystic echinoeoccosis （CE） and alveolar echlnoeoccosis （AE）. Surgical operation has been the main therapeutic method of eehinocoeeosis. But relapse always occurs. Therefore, chemotherapeutic research rose up at home and abroad in the recent decades. Mebendazole （MBZ） and albendazole （ABZ） are the best anti-echinoeoceal drugs, especially the former was replaced by the latter due to poor intestinal absorption. Self-designed Xiao-bao capsule was comprised of areca, ornphaia, snake slough, ground beetle, honeycomb and gangolin scales. Each capsule contains 250 mg of pow- der. The results of mice experiment and clinical trial indicated that Xiao-bao capsule exhibited good efficacy with very low toxicity. The clinical observation on 9 eases of liver CE and 1 ease of lung CE treated by com- bined treatment was summarized in this paper, appended with the ultrasonic comparison of pre- and post-treat- ment. The ultrasonic figures showed that in the beginning mixed echo group turned to heterosenous, later homogeneity became gradually with occasional multiple calcification, indicating a cure stage.
