摘 要:目的4-1BBL(CD137L)为肿瘤坏死因子超家族成员,4-1BBL与其受体4-1BB结合为T细胞活化提供共刺激信号。有研究证实激动性4-1BBL抗体能降低过敏性哮喘小鼠的气道高反应性,减少气道炎症细胞浸润。但4-1BBL在过敏性哮喘患者中的作用尚不清楚。方法我们比较了过敏性哮喘患者和健康对照组外周血单个核细胞中4—1BBL mRNA的表达水平和血浆中可溶性4-1BBL(soluble 4—1BBL,s4-1BBL)的浓度,并分析了外周血4—1BBL与哮喘病情的关系。结果结果为哮喘患者血浆s4IBBL的浓度(223.47±69.28)ug/L,正常对照组s4-1BBL浓度为(303.12±41.48)ug/L,哮喘组较正常组显著下降(P〈0.05),而外周血单个核细胞4—1BBLmRNA水平哮喘组与正常对照组之间差异无统计学意义(P〉0.05);哮喘患者s4—1BBL的浓度与血浆屋尘螨特异性IgE无相关性(r=0.072,P〉0.05),但与患者FEV,占预计值%呈显著负相关性(r=-0.487,P〈0.05)。结论s4—1BBL共刺激分子在过敏性哮喘的免疫反应中可能发挥重要作用,s4—1BBL减少可导致体内免疫反应失衡而促进哮喘的发生。Objective 4- 1BB ligand (4-1BBL,CD137L) is a member of the tumour necrosis factor superfamily expressed primarily on antigen presenting cells such as B cells, macrophages and dendritic cells. Its engagement with the receptor 4-1BB(CD137) has been shown to promote T-cell activation and regulate proliferation and survival of T cells. In murine allergic asthma model,it is shown that stimulation of the agonistic 4- 1BBL mAh prevent the development of airway hyperreactivity and airway inflammation cells infiltration, but the role of the costimulatory molecule in patients with allergic asthma remains unclear. Methods In this study, the expression of 4 -1BBL mRNA and soluble 4 -1BBL (s4-1BBL) protein concentration were analyzed in peripheral blood of patients with asthma. Compared with healthy controls, and analyzed the relationship between peripheral blood 4-1BBL and asthma severity. Results Result show the plasma s4-1BBL protein concentration of patient with asthma(223.47 ± 69.28) ug/L, hut healthy control as ( 303.12± 41.48) ug/L, patient' s plasma s4 -1BBL protein concentration decreased significantly comparing with healthy control ( P 〈0.05), but we didn't find the differenee on the expression of 4-1BBL mRNA in PBMC with real time PCR method( P 〉0. 05). In addition to,the study demonstrated plasma s4- 1BBL and house dust mite specific IgE had no correlation ( r= 0.072, P 〉 0.05), but showed significant negative with FEV1 percentage of predicted value( r = -0. 487, P 〈0.05). Conclusions We speculated s4 -1BBL costimulatory molecules play an important role in patients with asthma on the immune response, its decreasing induced to immune response imbalances and promote the occurrence of asthma.
