噬菌体生物扩增法检测结核杆菌在糖尿病合并结核感染快速诊断中的应用

摘　　要：[目的]探讨噬菌体生物扩增法检测结核杆菌在糖尿病合并结核感染快速诊断中的应用价值。[方法]收集本院结核病区及门诊糖尿病患者的痰、支气管镜镜抽取液标本共89份,应用噬菌体生物扩增法检测结核分支杆菌,同时作抗酸染色、并用VersaTREKTM细菌培养仪做分支杆菌培养。噬菌体生物扩增法和/或培养阳性的菌株32株做分支杆菌菌种鉴定。[结果]噬菌体生物扩增法检测的阳性率31.4%(28/89)显著高于涂片法19.1%(17/89)(P﹤0.01),与VersaTREKTM培养法阳性率28.1%(25/89)相比,差异无统计学意义(P﹥0.05)。21株VersaTREKTM培养法与噬菌体生物扩增法同时阳性的菌株和7株噬菌体生物扩增法阳性菌株均鉴定为结核分支杆菌;4株VersaTREKTM培养法阳性菌株鉴定为非结核分支杆菌。[结论]噬菌体生物扩增法检测结核分支杆菌快速、特异、敏感,在糖尿病合并结核感染的快速诊断中有重要价值。[Objective] To evaluate the value of rapid detection for mycobacterium tuberculosis with phage biological amplification in patients complicated with diabetes and tuberculosis infection.[Methods] 89 clinical specimen of sputum and bronchoscopic abstracts obtained fluid from the patients were detected by phage biological amplification,acid-fast staining and VersaTREKTM bacterial culture.The positive 32 strains by phage biological amplification and VersaTREKTM culture were to make mycobacterium identification.[Results] The positive rates with phage biological amplification,smears test,and VersaTREKTM bacterial culture were 31.4%（28/89）,19.1%（17/89）,and 28.1%（25/89）,respectively.The positive rate with phage biological amplification was significant higher than that of smears test（P﹤0.01）.There was no difference between phage biological amplification and VersaTREKTM culture（P﹥0.05）.21 strains from the positive 25 strains by VersaTREKTM culture were identified to be mycobacterium tuberculosis and the specificity was 84.0%.As for phage biological amplification,28 strains were all identified to be mycobacterium tuberculosis and the specificity was 100%.[Conclusion] The phage biological amplification for mycobacterium tuberculosis is rapid,specific and sensitive,so it is important in the rapid detection of patients complicated with diabetes and tuberculosis infection.
