手法无缝线小切口白内障摘除术中并发症临床分析

摘　　要：目的:分析手法无缝线小切口白内障摘除术(MSICS)中并发症的发生原因,探讨其处理方法。方法:对142眼施行MSICS,回顾其术中出现的并发症并加以分析。结果:术中后囊膜破裂15眼(10.6%);上方虹膜反复脱出9眼(6.3%);角膜水肿2眼(1.4%);虹膜根部离断11眼(7.7%),其中下方虹膜根部离断7眼,上方虹膜根部离断4眼;前房积血1眼(0.7%);上方切口渗漏11眼(7.7%)。本组病例术中无晶状体核坠入玻璃体腔和暴发性脉络膜上腔大血发生。结论:掌握手术适应证,规范、熟练的手术操作方法和技巧可降低术中并发症发生率,及时的处理术中并发症可以获得满意的手术效果。Objective：To analyze the cause and treatment of complication during manual small-incision cataract surgery（MSICS）.Methods：Clinical data of 142 eyes with MSICS were collected and analyzed.Results：During surgery,15 eyes（10.6%）had envelop rupture,9 eyes（6.3%） had repetitive prolapse of upper iris,2（1.4%） had comeal edma,11（7.7%） had mutilation of iris root including 7 lower iris root mutilation and 4 upper,1（0.7%） had anterior chamber hyphema and 11（7.7%） had upper incision leakage.There was no case with lens nucleus falling in vitreous cavity or epichoroidal explosive hemorrhage.Conclusions：Correct surgical indication,normalized and skilled surgery procedure and technique can decrease incidence of complication.Besides,handling complication in time is helpful in successful surgery
