摘  要:目的 探讨妊娠期急性胰腺炎(acute pancreatitis in pregnancy,APIP)并发急性呼吸窘迫综合征(ARDS)危险因素可能的独立危险因素.方法 选择2000-01 ~2010-04连续收治资料完整的妊娠期急性胰腺炎62例,根据住院一周内是否发展为ARDS分为ARDS组和非ARDS组,回顾性分析两组间相关临床指标的差异,多因素Logistic回归分析筛选其并发ARDS的独立危险因素.结果 外周血淋巴细胞计数、血小板计数、血钙、碱剩余(BE)和尿素氮(BUN)在ARDS组和非ARDS组差异有统计学意义;BE降低 (优势比1.782;95%可信区间CI 1.031 ~3.210;P=0.015)和血清BUN升高(优势比3.759;95%可信区间CI 2.512~6.023;P=0.000)是妊娠期急性胰腺炎并发ARDS的独立危险因素.结论 妊娠期急性胰腺炎患者早期监测动脉血BE和血清BUN有利于预后预测和病情评估.Objective To investigate independent risk factors of acute pancreatitis and acute respiratory distress syndrome (ARDS) in pregnancy. Methods All 62 patients were admitted to our hospital from January 2000 to April 2010. According to the prognosis within a week, all the patients were divided into non - ARDS group and ARDS group, and independent risk factors for acute panereatitis and ARDS patients in pregnancy were screened by multivariate Logistic regression analysis between the two groups. Results There were significant differences in peripheral blood lymphocyte count, platelet count, serum calcium, base excess and blood urea nitrogen between non - ARDS group and ARDS group. Decrease of base excess [ odds ratio 1. 782 ; 95% confidence interval (CI) 1. 031 - 3. 210; P = 0. 015 ] and increase of blood urea nitrogen ( odds ratio 3. 759 ; 95% CI 2. 512 - 6. 023 ; P = 0. 000) were independent risk factors for acute pancreatitis and ARDS patients during pregnancy. Conclusion Arterial base excess and serum urea nitrogen may play an important role in condition assessment and prognosis prediction ibr acute pancreatitis patients in pregnancy.
