1000例动态心电图的Lorenz散点图分析

摘　　要：目的探讨Lorenz散点图在动态心电图中的诊断价值。方法将1000例24h动态心电图数据做成Lorenz散点图,将其图形与动态心电图诊断及心率变异传统指标进行对比分析。结果①心律失常类型与Lorenz散点图图形特点的对比分析:异位心搏<100次共483例,其中391例呈一分布图形;异位心搏100次共517例,415例呈多分布图形。Lorenz散点图图形特点与动态心电图诊断有很好的对应关系;②非线性分析(Lorenz散点图)和线性分析(时域、频域分析)对比:HRV在正常范围内共405例,"彗星状"占77.28%;频域降低共155例,"鱼雷状"占80%;出现"短棒状"时时域、频域均降低,占94.28%。结论 Lorenz散点图形特点与心律失常类型、HRV线性分析指标相关性较好。Objective To discuss the values of Lorenz plot of the ambulatory electrocardiogram（AECG） diagnosis.Methods Lorenz plots were made from the data of 24h ambulatory electrocardiograms of 1000 patients. Then these LPs were analyzed in contrast with the diagnosis of ambulatory electrocardiograms and the traditional analytical method of HRV.Results①The contrast analysis between different kinds of arrhythmia and the characteristics of Lorenz plot：There were 483 cases whose arrhythmic frequency100 times per 24h,in which 391cases＇LPs were one distribution pictures;another 517cases whose arrhythmic frequency100 times per 24h,in which 415 other cases＇were more than one distribution pictures.The characteristics of Lorenz plot has good corresponding relation to the diagnosis of AECG.②Contrast analysis between Nonlinear（LP）and linear analysis （frequency domain analysis and time domain analysis）：405 cases＇HRV were in the normal range,77.28%LP were in the shape of ＂comet＂ ;155 cases＇frequency domain analysis were reduced,and 80%LP were in the shape of ＂torpedo＂ ;When the LP were in the shape of ＂cosh＂ ,94.28%cases have both frequency domain analysis and time domain analysis reduced.Conclusion The characteristics of Lorenz plot has good corresponding relation to the arrhythmia diagnosis of AECG and HRV linear analysis.
