甘肃省乙型肝炎疫苗接种与乙型肝炎病毒感染关系探讨

摘　　要：[目的]分析乙肝疫苗接种和乙肝病毒感染的关系,为今后控制乙肝提供参考。[方法]通过多阶段随机抽样的方法,抽取1～59岁的自然人群,对每名对象的接种情况进行问卷调查,并抽取1ml血清,利用ELASA方法检测HBsAg、抗-HBs和抗-HBc,对接种情况与感染情况进行相关性分析。[结果]随着年龄的升高,1～14岁儿童的首针及时接种率和全程合格接种率有逐步下降的趋势,而感染率有升高的趋势。进行相关性分析,首针及时接种率和乙肝感染率之间呈负相关,相关系数为-0.598,P﹤0.05;全程合格接种率与乙肝感染率无相关性,P﹥0.05。考虑到有可能是首针及时接种率的影响,排除首针及时接种率的干扰,重新计算全程合格接种率与乙肝感染率的相关性,P﹥0.05,由此看来全程合格接种率与乙肝感染率之间不存在相关关系。[结论]提高乙肝疫苗首针及时接种率对控制乙肝至关重要。[Objective] To analyze relationship between Hepatitis B vaccination and infection rate of Hepatitis B virus in Gansu Province, and provide the reference for controlling hepatitis B in Gansu Province in the future. [Methods] HBsAg, anti-HBs and anti-HBc were tested with ELASA and questionnaire survey about Hepatitis B vaccination was carried out among population aged 1-59 years old selected by multi-stage random method, and then analyzed correlation between them. [Results] As age increases, there were a downward trend of timely birth dose hepatitis B vaccination coverage rate and whole qualified vaccination coverage rate and a rising trend of infection rate in children aged 1-14. It was a negative correlation between timely birth dose hepatitis B vaccination coverage and infection rate, and the correlation coefficient was -0.598, P﹤0.05. There was no correlation between whole vaccination coverage rate and infection rate, P﹥0.05. Taking timely birth dose coverage into account, the correlation between whole vaccination coverage and infection rate was re-calculated excluding interference of timely birth dose coverage, and still showed no correlation, P﹥0.05. [Conclusion] It is essential for hepatitis B control to increase timely birth dose hepatitis B vaccination coverage.
