摘 要:目的:探讨64层CT血管造影(CTA)在主动脉夹层及主动脉瘤中的临床应用价值。方法:收集2009年1~12月经64层CT主动脉血管造影诊断主动脉夹层和主动脉瘤21例患者,对所得数据进行最大密度投影(MIP)、多平面重建(MPR)、曲面重建(CPR)、血管分析(VA)、容积再现(VR)D等后处理重建,所有诊断结果均经手术证实。结果:21例患者中,主动脉夹层18例,主动脉瘤3例,多种后处理重建方法结合使用,能对主动脉夹层破口的位置、分型、波及范围、真假腔、撕破的内膜、夹层的旋转方向、血栓形成等情况进行准确的显示,对主动脉瘤的位置、范围、类型、血栓形成等情况以及瘤壁钙化清晰显示。结论:64层螺旋CT血管成像操作简单,速度快、创伤小,以横断面影像为基础,结合其他二维、三维成像方法,可以更准确、直观地显示主动脉夹层及主动脉瘤的部位、大小和范围。为临床制定治疗方案提供依据,有重要的临床应用价值。Objective: To evaluate the clinical application value of 64-slice CT angiography (CTA) in the diagnosis of aortic dissection and aortic aneurysm.Methods:21 patients with aortic dissection or aortic aneurysm underwent CTA, the post-processing techniques such as maximum intensity projection (MIP), multiplanar reconstruction (MPR), surface reconstruction (CPR).vessel analysis (VA), volume rendering (VR) were made,all the diagnosis confirmed by surgery.Results:21 patients,18 cases of aortic dissection, aortic aneurysm in 3 cases,A variety of reconstruction in combination can show the the location of aortic dissection,type,affected scope,true and false lumen, the endometrium, the direction of rotation sandwich, thrombosis, etc.as well as the location of the aortic thrombosis,wall calcification,etc.Concluslons:64 slice CTA is a simple, fast, minimally invasive technique, the application of cross-sectional images, combined with other two-dimensional, three-dimensional imaging method can accurately display the location,size and scope of aortic distrction and aneurysm, It can provide the basis for clinical treatment plan and has important clinical value.
