经皮气管旋切术在危重患者人工气道建立中的应用价值

摘　　要：目的:探讨经皮气管旋切术(PDT)在危重患者人工气道建立中的应用价值。方法:38例患者行PDT,观察其手术情况,围手术期及后期并发症,切口愈合时间,手术安全性等,并与传统的开放式气管切开术(OT)进行比较。结果:PDT组的手术时间(9.5±6.3)分钟,术中出血量(11±9.5)ml,切口长度(1.45±0.15)cm,切口愈合时间(4±2)d;OT组手术时间(26.60±8.15)min,术中出血量(25 5±12.2)ml,切口长度(4.1±0.32)cm,切口愈合时间(9±2)d。两组上述指标比较差异均有统计学意义(P<0.05)。结论:PDT组手术时间短,愈合快,出血量少,围手术期和后期并发症发生率低,该术式是一种微创的、快捷的急救技术,适用于需建立人工气道的危重患者。Objective：To evaluate the percutancous dilatational tracheotomy （PDT） in critically ill patients with artificial airway to establish the application.Methods：38 patients underwent PDT, observe the operation conditions of perioperative and late complications,wound healing, surgery safety,etc.,and with traditional operational tracheostomy （OT） were compared.Results：PDT group,the operation time （9.5±6.3 minutes）,blood loss （1 1±9.5）ml,incision length （1.45±0.15）era,wound healing time （4±2）d;OT operation time （26.60±8.15）rain,blood loss （25.5±12.2）ml,incision length （4.1±0.32）era,wound healing time （9±2）d.The target groups were statistically significant differences （P〈0.05）. Conclusioa：PDT group operation time is short,fast healing,less blood loss,perioperative and late complication rate is low, the repair is a minimally invasive,fast first-aid techniques,appropriate for the establishment of artificial airway of the critically ill patients.
