卵巢巧克力囊肿破裂18例临床特点分析

摘　　要：目的:探讨卵巢巧克力囊肿破裂的临床表现及误诊原因,以提高确诊率。方法:回顾性分析2002年11月至2009年1月在我科住院治疗的18例经手术、病理证实为卵巢巧克力囊肿破裂患者的临床资料。结果:18例患者中,术前确诊为卵巢巧克力囊肿8例,B超检查均提示有盆腔囊性肿物,并经后穹隆穿刺抽出淡咖啡色血性液体,确诊率为44.44%;误诊10例,误诊率为55.56%。结论:通过详细询问病史、体格检查,辅以后穹隆穿刺、B型超声检查、血CA125的检测可提高卵巢巧克力囊肿术前确诊率,一旦确诊应尽早行手术治疗。Objective： To evaluate the clinical manifestations of the ovarian chocolate cyst rupture and the cause of the misdiagnosis,improve accuracy of diagnosis.Methods： The clinical data of 18 hospitalized patients with confirmed ovarian chocolate cysts rupture in our department from November 2002 to January 2009 were retrospectively analysed.Results： 18 patients,8 cases of ovarian chocolate cyst were preoperatively diagnosed,B-ultrasonic examinations were suggestive of pelvic cystic mass,and with light brown bloody fluid after culdocentesis,the diagnosis rate was 44.44%;10 cases were misdiagnosed,misdiagnosis rate was 55.56%.Conclusion： The detailed history,physical examination,supplemented of culdocentesis,B-ultrasound,CA125 blood test can increase the rate of preoperative diagnosis,once the diagnosis is confirmed,the operation should be done as early as possible.
