摘  要:目的 检测HBV感染者血清中乙肝表面抗原大蛋白(LHBS),DANE颗粒,HBV-DNA和PreS1蛋白,探讨LHBS在临床诊治中的价值.方法 对1 042例HBV感染者采用酶联免疫吸附试验检测乙肝病毒HBeAg,LHBS,DANE颗粒和PreS1蛋白,并采用定性PCR检测HBV-DNA.结果 245例HBeAg(+)的HBV感染者中,LHBS阳性率(96.33%),DANE颗粒阳性率(90.20%),分别与HBV-DNA阳性率(92.65%)比较,差异无统计学显著性意义(P>0.05),三者均明显高于PreS1阳性率(66.53%),差异均有统计学显著性意义(P<0.01);797例HBeAg(-)的HBV感染者中,LHBS阳性率(78.17%)高于DANE颗粒阳性率(56.46%),HBV-DNA阳性率(33.12%)和PreS1阳性率(17.44%),依次比较差异均有统计学显著性意义(P<0.01).结论 LHBS是判断HBV感染者体内病毒复制程度的可靠指标,在HBeAg(-)的HBV感染者中,LHBS有指导治疗、预后判断的作用;DANE颗粒亦是判断病毒复制程度的可靠的新指标.LHBS和DANE可用于暂无条件开展PCR实验的的基层医疗单位,代替HBV-DNA检测.Objective To evaluate the significance of the serum Hepatitis B virus large surface protein detection to HBV patients. Methods LHBS,DANE particle and PreS1-Ag were detected with ELISA while HBV-DNA with PCR in 1 042 serum samples collected from HBV patients. Results In 245 sample from HBeAg(+) serum,there was no significant difference between the positive rate of LHBS (96.33 % ), DANE particle (90.20% ) and HBV-DNA (92.65 % ), (separately, P〉0. 05) ;PreS1-Ag (66. 53% ) was lower than others significantly(separately,P〈0.01). In 797 samples from HBeAg(-) serum, the positive rate of LHBS (78.17%) was significantly higher than DANE particle (56.46%), HBV-DNA (33.12%) and PreS1-Ag (17.44%), (separately,P〈0. 01). Conclusion It is suggested that LHBS may be served as reliable marker of reflecting HBV replication. LHBS is valuable to monitor the treatment and diagnosis of the patients with HBeAg(- ) chronic hepatitis B. DANE particle also could reflect HBV replication. LHBS and DANE particle could be used by the basic medical infirmary to replace PCR which is expensive and complicated.
