摘  要:目的:了解上海市闵行区2007年-2009年度人群流感的流行特征,为本地区流感预防控制工作提供科学依据。方法:收集、汇总闵行区流感监测哨点的流感样病例数、就诊人数等数据进行描述性流行病学分析;对采集的流感样病例咽拭子标本用狗肾传代细胞进行流感病毒分离、培养,用红细胞凝集抑制实验鉴定流感病毒型和亚型。结果:2007年-2009年监测到流感样病例总数为7938例,流感样病例平均就诊比例为0.52%。,采集标本1277份,分离到各型流感毒株406株。2007年优势株为季节性H3N2,2008年优势株为季节性H1N1与B型(Yamagate系)共同流行,病毒分离及流行病学监测都显示2007年-2008年闵行区流感流行大致呈冬春季和夏季高发的特点。2009年冬春、夏季流感发病高峰不明显,但在11月-12月出现全年流感流行高峰,病毒分离结果显示冬春季为季节性H3N2、H1N1共同流行,夏秋季开始以季节性H3N2、甲型H1N1在人群中同时流行,之后以甲型H1N1为主。结论:2007、2008年闵行地区流感流行特点为冬春季和夏季高发,病毒优势株呈季节性H3N2、B、H1N1交替流行态势。2009年秋冬季暴发甲型H1N1流感疫情,9月-12月甲型H1N1毒株占全年检出毒株数的50%.Objective:To understand the influenz epidemic characteristics of population in Minhang from 2007 to 2009,to provide scientific bases for the prevention of influenz.Methods: Collecting surveillance data of influenza-like illness(ILI) in monitored hospital of Minhang.All taking specimens were processed to culture and isolate the influenz virus in MDCK cell,and identified type and subtypes using the haemagglutination inhibition test(HI).Results: Totally 7938 cases ILI were reported in monitored hospital from 2007 to 2009,the doctor ratio of cases ILI were 0.52%.Taking specimens were 1277,406 strains of influenz virus were isolated.The dominant epidemic strain of influenz virus was seasonal H3N2 in 2007,seasonal H1N1 and B co-circulated in 2008.Both of influenz surveillance and identification of influenz virus showed that the high incidence of influenz was in winter-spring and summer from 2007-2008.The two seasonal influenza peaks of winter-spring and summer were obvious in 2009,but the epidemic peak of influenz was apparent in November and December,The seasonal influenza H1N1 and H3N2 co-circulated in the winter-spring.In summer-autumn,influenza A(H1N1) and the seasonal H3N2 co-circulated in Minhang,then influenza A(H1N1) became dominating.Conclusion: The epidemic peaks of influenza were winter-spring and summer-autumn in Minhang Area from 2007 to 2008,the dominant strains was alternation between H3N2,B and H1N1.The outbreak caused by influenza A(H1N1) was reported in autumn-winter in 2009,the detection ratioe of Influenza A(H1N1) virus was 50%.
