天花粉治疗异位妊娠过程中血β-HCG浓度曲线的动态变化

摘　　要：目的:测定用天花粉保守治疗异位妊娠患者血中的β-HCG,并画出其浓度曲线的动态变化,来了解用天花粉治疗异位妊娠用药的最佳时期和治疗时间。方法:选择本院2008年9月-2009年10月收住入院确诊为异位妊娠用天花粉蛋白注射治疗的299例患者,按治疗期间血β-HCG升高与降低的不同分为A、B两组,A组:第1次用2.4 mg天花粉蛋白注射后血β-HCG值上升超过用药前水平,于第2次用1.2 mg天花粉蛋白注射后逐渐下降。B组:第1次用2.4 mg天花粉蛋白注射后血β-HCG值呈逐渐下降。结果:首次血β-HCG值大于5000 mIU/ml有32例占10.7%,小于500 mIU/ml有47例占15.7%,在500 mIU/ml～5000 mIU/ml有220例占73.5%,本次实验选择首次浓度在500 mIU/ml～5000 mIU/ml已经治疗成功的220例作为研究对象,然后画出血β-HCG浓度曲线,观察曲线的动态变化。结论:利用血β-HCG浓度曲线的动态变化能掌握首次注射天花粉治疗异位妊娠的敏感性,对血β-HCG值上升超过用药前水平要补注天花粉蛋白应尽量集中在血β-HCG值下降至峰值50%之前时使用治疗效果最佳,总疗程比首次用药既敏感的治疗时间要长。Objective：Measure the serum β-HCG on ectopic pregnancy patient treated with Variola Powder Protein.Draw up the concentration curve,to know the optimized period of medication.Methods： 299 ectopic pregnants admitted from Sep 2008 to Oct 2009 and treated with Variola Powder Protein were analyzed retrospectively.The patients were divided into two groups according to the level of serum HCG.Group A： After first injection with 2.4 mg Variola Powder Protein,serumβ-HCG rise exceed the level before medication.After second injection with 1.2 mg Variola Powder Protein,serumβ-HCG fall-off.Group B： After first injection with 2.4 mg Variola Powder Protein,serumβ-HCG fall-off. Results： 32 patients whose serum HCG exceed 5000 mIU/ml account 10.7%,47 patients serum HCG500 mIU/ml account 15.7%,220 patients serum HCG between 500 mIU/ml～5000 mIU/ml account 73.5%.We chose the 220 patients as our research object,draw the β-HCG concentration curve.To analyze the dynamic change of the curve.Conclusion： We can know the therapy sensibility of Variola Powder Protein on ectopic pregnancy by the β-HCG concentration curve.To those patient whose serumβ-HCG rise exceed before Medication,we should give them another dose.It is better to use the drug before serumβ-HCG descending to half of the peak.The total course of treatment is longer than that of sensitive patients when give them fist dose.
