慢性丙型肝炎患者血清HCVRNA病毒载量与肝损伤程度的探讨

摘　　要：目的:探讨丙型肝炎患者血清HCVRNA病毒载量与肝脏功能生化指标变化的关系。方法:对180例慢性丙型肝炎患者和30例健康体检者进行分组,其中A组30例为健康体检者,B组60例为抗-HCV阳性,HCVRNA含量<1×103拷贝/毫升,C组80例为抗-HCV阳性,HCVRNA含量在1×103～9.99×105拷贝/毫升,D组40例为抗-HCV阳性,HCVRNA含量在≥1×106拷贝/毫升。对210例标本同时进行抗-HCV、HCV-RNA定量、肝功能(ALT、AST)检测,分别比较各组间关系。结果:A组30例体检者ALT正常27例异常3例,AST均正常。B组60例ALT正常51例异常9例,AST正常55例异常5例。C组80例ALT正常20例异常60例,AST正常27例异常53例。D组40例ALT正常3例异常37例,AST正常5例异常35例。A组与B组之间肝功能比较差别无统计学意义(P>0.05),A组、B组分别与C组、D组之间肝功能比较差别有统计学意义(P<0.05),C组与D组之间肝功能比较差别有统计学意义(P<0.05)。结论:慢性丙型肝炎血清HCVRNA病毒复制水平与肝脏功能ALT、AST异常表达相关,慢性丙型肝炎肝功能异常最直接因素可能为HCVRNA的复制所导致。Objective：Explore patients with hepatitis c virus and HCVRNA serum biochemical changes of liver function.Methods： For 180 cases of chronic hepatitis C and the healthy check-up of 30 cases of group,A group of 30 cases of healthy check-up,B group of 60 cases for positive,HCVRNA 1×103 copy/ml,C group of HCVRNA 1×103～9.99×105 copy/ml D group is positive,HCVRNA 1×106 copy/ml.At the same time to 210 cases,HCV,HCV-RNA quantitative and liver（ALT）,respectively,the AST comparison between groups.Results： A group of 30 cases of 27 patients medical check-up ALT normal abnormal 3 cases,the AST.B group of 60 patients with normal ALT 51 cases anomalies in 9 cases,55 patients with abnormal AST normal 5 cases.80 cases of group C 20 cases of normal ALT abnormal 60 patients,27 AST normal abnormal 53 cases.D group 40 cases of normal ALT 37 cases,3 cases of abnormal AST normal abnormal 35 patients in 5 cases.Group A with B group（P0.05 compared between liver earthqyuake is A group project,respectively,group B and C,D group（P0.05 compared between liver,with A statistical significance,the group C group D withcomparison between the liver function was statistically significant（P0.05）.Conclusion： Chronic hepatitis c virus replication HCVRNA serum ALT levels and liver function abnormal AST,positively correlated,chronic hepatitis c abnormal liver function is the most direct factors for HCVRNA copy may be caused.
