2009年我院医院感染病原菌的临床分布及耐药性分析

摘　　要：目的:了解2009年我院医院感染病原菌的分布特点及其对常用抗菌药物的耐药状况,为医院感染的预防与控制提供参考。方法:采用BACTEC 9120细菌培养仪及PHN IX100全自动细菌鉴定仪对临床分离菌进行鉴定及药物敏感试验。结果:共分离医院感染病原菌1925株,其中革兰阴性杆菌1161株,占60.3%,主要是:铜绿假单胞菌、大肠埃希菌、肺炎克雷伯氏菌、不动杆菌属。肠杆菌科细菌对头孢哌酮/舒巴坦、亚胺培南和阿米卡星的敏感率均在75%以上。铜绿假单胞菌对头孢哌酮/舒巴坦和亚胺培南的耐药率分别为17.2%和31.3%。革兰阳性球菌562株,占29.2%;主要以葡萄球菌为主,其中MRSA(耐甲氧西林金黄色葡萄球菌)和MRCNS(耐甲氧西林凝固酶阴性葡萄球菌)的检出率分别为42.4%和66.0%。真菌占10.5%,以白色念珠菌为主,暂时没有进行常规的药敏检测。结论:2009年我院医院感染病原菌以铜绿假单胞菌和金黄色葡萄球菌为主,非发酵菌的检出率较高,且已经呈现多药耐药.需加强对医院感染的认识,采取相应预防措施,控制和减少感染的发生。Objective：To study pathogens distribution and their drug-resistance characteristics of the bacterial infection in our hospital in 2009 so as to offer a reference for prevention and control of nosocomial infectiong.Methods： The clinical isolates were identified with BACTEC 9120 or PHN IX100.Results： A total of 1925 nosocomial pathogens were collected in 2009.Gram-negative bacilli accounted for 60.3%.The main pathogens were Pseudomonas aeruginosa,Escherichia coli,Klebsiella pneumoniae,A cinetobacter spp.For Enterobacteriaceae strains,the susceptibility rates to cefoperazone/ sulbactam,imipenem and amikacin were found to be above 75.About 17.2% and 31.3 % of P.aeruginosa isolates were resistant to cefoperazone/ sulbactam and imipenem.Gram-positive cocci accounted for 29.2%,most of them were staphylococci.About 42.4 % of S.aureus isolates were MRSA,66.6% of coagulase-negative Staphy lococcus isolates were meticillin-resistant.Fungi accounted for 10.5 %.Most of them were Candida albicans.The susceptibility tests have not be regularly performed for fungi at present.Conclusion： P.aerugionosa and S.aureus are the main pathogens to cause nosocomial infection in our hospital showing multi-drug-resistance during 2009.To emphasize on the recognition of the nosocomial infections and the drug-resistance,it is important to be taken certain preventive measure.
