摘　　要：目的系统评价匹伐他汀对比阿托伐他汀治疗原发性高胆固醇血症的疗效与安全性。方法计算机检索Cochrane图书馆(2010年第1期)、PubMed、EMBASE、中国知网、维普资讯网、中国生物医学文献数据库、万方等中外生物医学数据库,纳入匹伐他汀对比阿托伐他汀治疗原发性高胆固醇血症的随机对照试验,检索日期截止至2010年4月,按照Cochrane系统评价方法,评价纳入研究的方法质量学,并提取有效数据采用RevMan5.0软件对两组患者的低密度脂蛋白胆固醇、总胆固醇、高密度脂蛋白胆固醇、甘油三酯的变化水平及不良反应发生率等指标进行Meta分析。结果共纳入了9个随机对照试验,共1733名病例,Meta分析结果表明:两组患者分别在治疗第4周、第8周、第12周后低密度脂蛋白胆固醇、总胆固醇、高密度脂蛋白胆固醇、甘油三酯的变化水平及不良反应发生率均无差异。结论目前的证据表明:匹伐他汀具有与阿托伐他汀相似的降脂效果和安全性,且具有服用剂量低的优势,但其疗效方面的长期优势值得进一步研究。Objective To systematically evaluate the clinical effectiveness and safety of Pitavastatin versus Atorvastatin in the treatment of patients with hypercholesterolemia.Methods We electronically searched PubMed,EMBASE,Cochrane Library（2010,issue 1）,CBM,CNKI,VIP and WANFANG databases to April 2010.Randomised controlled trials（RCTs）were indentified about Pitavastatin versus Atorvastatin in the treatment of patients with hypercholesterolemia.Study selection and analysis were undertaken according to the Cochrane Handbook,and RevMan 5.0 was applied for statistical analysis.The following was studied：the decrease of blood levels of LDL-C,TC,TG;the increase of blood levels of HDL-C and adverse events.Results Nine RCTs involving 1 733 patients were included.Meta analyses indicated that there were no significantly differences between Pitavastatin group and Atorvastatin group of blood levels of the decrease of LDL-C,TC,TG;the increase of HDL-C and adverse events.Conclusion The current evidence available shows that the Pitavastatin has the same efficacy compared to the Atorvastatin,both the two treatments are well tolerated.
