穴位注射加微波对PHN治疗前后血浆的β-内啡肽水平的影响

摘　　要：目的:探讨穴位注射加微波治疗带状疱疹后遗神经痛(PHN)的疗效及治疗前、后患者血浆β-内啡肽水平的影响。方法:将86例带状疱疹后遗神经痛患者随机分为治疗组和对照组,每组43例。治疗组穴位注射加微波治疗,对照组仅采用微波治疗,共5周。治疗前、后采用视觉模拟评分法(VAS)测定疼痛强度标准评分,以放射免疫法测定血浆β-内啡肽水平。选择与PHN患者的年龄、性别匹配的同期在我院行健康体检的10例健康人为正常对照组。结果:两组患者治疗前VAS值比较差异无统计学意义(P>0.05);治疗后VAS值比较差异有统计学意义(P<0.05)。血浆β-内啡肽水平两组治疗前血浆β-内啡肽水平与正常对照组比较差异有统计学意义(P<0.01);治疗后治疗组、对照组血浆β-内啡肽水平与各自治疗前比较,差异有统计学意义(P<0.01),且治疗后治疗组低于对照组,差异有统计学意义(P<0.01)。结论:穴位注射加微波治疗可以调节带状疱疹后遗神经痛患者血浆β-内啡肽水平,且影响免疫细胞的活性,导致多种炎症介质的产生减少,从而达到治疗效果。Objective：To investigate the effects of acu-point injection and microwave treatment on postherpetic neuralgia（PHN） and their influence on the plasma β-endorphin level.Methods：A total of 86 patients with postherpetic neuralgia were randomly divided into treatment group and positive control group with 43 cases in each group.The treatment group was treated with acu-point injection and microwave therapy,while the control group was only treated with microwave therapy for 5 weeks.Before and after treatment,pain intensity of the two groups was evaluated using a visual analogue scale（VAS） and the plasma β-endorphin levels were measured by radioimmunoassay.Ten age and gender-matched healthy volunteers were selected as control.Results：Before treatment,no significant difference in VAS values was observed between the treatment and positive control group（P0.05）;while statistically significant difference presented after treatment（P0.05）.Before treatment,the plasma β-endorphin levels of both the treatment and the positive control group were higher than that in the health control volunteers（P0.01）.After the treatment,the plasma β-endorphin levels decreased significantly in both groups,but more obviously in the treatment group（P0.01）.Conclusions：Acu-point injection and microwave treatment can regulate the plasma β-endorphin levels in postherpetic neuralgia patients and shows influence on activity of immune cells,resulting in reduction of various inflammatory mediators production.
