冷循环射频消融治疗中晚期肝癌的近期疗效评价

摘　　要：目的:评价彩超引导下冷循环射频消融治疗中晚期肝癌的近期疗效。方法:中晚期肝癌30例,其中原发性肝癌21例,转移性肝癌9例。肿瘤最大径≤5 cm的12例肝癌患者采用单纯射频消融治疗(RFA),肿瘤最大径>5 cm的18例肝癌患者采用RFA联合肝动脉化疗栓塞术(TACE)治疗。RFA术后1个月行超声造影或肝脏增强CT、血清肿瘤标志物检测等以评价疗效。结果:30例患者完全灭活率46.7%(14例),部分灭活率53.3%(16例)。单纯RFA组12例完全灭活率50.0%(6例),部分灭活率50.0%;RFA联合TACE组18例完全灭活率44.4%(8例),部分灭活率55.6%(10例),两组无统计学差异(P>0.05)。15例AFP阳性患者术后AFP较术前显著下降(P<0.05)。结论:超声引导下冷循环射频消融治疗中晚期肝癌近期疗效较好,对于大肝癌宜联合TACE治疗。Objective：To evaluate the short-term efficacy of percutaneous radiofrequency ablation（RFA） with cool-tip electrode under ultrasound guidance on the treatment of advance liver cancer.Methods：A total of 30 patients of advance liver cancer were selected including 21 cases with primary hepatic carcinoma and 9 cases of metastatic liver cancer.Twelve patients（the largest tumor diameter ≤5 cm）received RFA therapy alone,18 patients（the largest tumor diameter 5 cm） had a combination therapy of TACE and RFA.The therapeutic effects were compared with contrast enhanced sonography or contrast enhanced computed tomography 1 month after RFA.Results：The ratio of complete inactivation in 30 advanced hepatic carcinoma cases was 46.7%（14 cases）,while the ratio of partial inactivation was 53.3%（16 cases）.The ratio of complete inactivation from RFA group（12 cases） was 50%（6 cases）,while the ratio of partial inactivation was 50%（6 cases）.The ratio of complete inactivation from RFA plus TACE group（18 cases） was 44.4%（8 cases）,while the ratio of partial inactivation was 55.6%（10 cases）.The difference was not significant in the efficacy between the two groups（P0.05）.Serum AFP levels（15 cases） were significantly decreased（P0.05）,as compared with that before treatment.Conclusions：Percutaneous RFA with cool-tip electrode under ultrasound guidance is effective on treating advance hepatic carcinoma,while RFA plus TACE treatment is recommended for large hepatic tumor.
