氩离子凝固术治疗食管上段胃黏膜异位症16例分析

摘　　要：目的：探讨内镜下氩离子凝固术（APC）治疗食管上段胃黏膜异位症（HGMUE）的临床疗效及内镜下诊断HGMUE发病率、检出率与内镜医师关注度、检查手法的关系。方法：对2007年4月~2010年7月发现的16例HGMUE患者在无痛胃镜下行APC治疗,并对其进行回顾分析。结果：16例患者经APC治疗后1~2月复查病灶处黏膜愈合较好,无柱状上皮残留。有效率达100%,未出现出血、穿孔等严重并发症。结论：内镜下APC治疗HGMUE,异位黏膜清除完全,效果确切,值得推广。Objective：To evaluate the clinical efficiency of endoscopic argon plasma coagulation（APC） in the treatment of upper oesophageal gastric heterotopia（HGMUE）,and to investigate how the detection rate of HGMUE under endoscope related to the operators′ attitude and skills.Methods：Sixteen HGMUE cases that were diagnosed as HGMUE and treated with APC under analgesia gastroscopy during April 2007 and July 2010 were retrospectively analyzed.Results：1-2 months after APC treatment,mucosal lesions healed well with no residual columnar epithelium in all the 16 cases with an effective rate of 100%,no complications like bleeding,perforation or other serious complications occurred.Conclusion：Endoscopic APC treatment,which can move the ectopic mucosa completely,is effective on HGMUE,and is worth for application in large scale.
