摘  要:目的:通过对危重症病人行早期肠内营养(EEN),观察病人的临床转归和评价EEN的应用价值。方法:将46例危重症病人随机分为治疗组和对照组。治疗组病人在入住ICU的24~48 h内行EN,对照组病人在入ICU的48 h后开始EN。比较两组病人对营养支持的耐受性、营养指标、肝功能、EN可耐受的起始时间、达EN目标喂养点所需时间、机械通气时间、平均ICU住院天数和28 d病死率等各项指标。结果:治疗组病人的前清蛋白、氮平衡和肝功能明显优于对照组(P〈0.05),EN可耐受的起始时间和达EN目标喂养点所需时间亦较对照组明显提前(P〈0.05)。治疗组病人的机械通气时间、平均ICU住院天数明显低于对照组(P〈0.05)。两组病人28 d病死率无显著性差异(P〉0.05)。结论:早期EN能及时纠正危重症病人的负氮平衡,改善病人的营养状况,缩短病人机械通气时间和平均ICU住院天数。Objective: To investigate the effect of early enteral nutrition support in treatment of critically ill patients.Methods: 46 patients who had been in ICU were randomly divided into experimental group and contral group.The experimental group accepted early enteral nutrition within 24-48 hours following admission to ICU.The results of nutrition support were analysed.Results: The differences of prealbumin,nitrogen balance and liver function were observed between the two groups(P0.05).Compared with the contral group,the time of target volume of the experimental group was shorter significantly(P0.05).The mechanically ventilated time and the average ICU stay were also notably shortened in the experimental group(P0.05).Conclusion: Early enteral nutrition can improve nutrition status,shorten mechanical ventilation time and the average ICU stay in critically in patients.
