摘　　要：目的:观察2型糖尿病(type 2 diabetes mellitus,T2DM)患者血清脂蛋白相关磷脂酶A2(lipoproteinassociated phospholipase A2,LP-PLA2)水平,探讨其与T2DM血管并发症的关系及临床意义。方法:选择180例T2DM患者,分为糖尿病合并冠心病组48例,糖尿病合并肾脏病变组59例,单纯糖尿病组73例。同期60名健康体检者为对照组。应用ELISA检测4组血清LP-PLA2水平,并进行比较。结果:糖尿病3组LP-PLA2水平与对照组比较差异均有统计学意义(P<0.05)。糖尿病合并冠心病组及糖尿病合并肾脏病变组LP-PLA2水平均高于单纯糖尿病组,差异有统计学意义(P<0.01)。LP-PLA2与低密度脂蛋白、体质量指数、空腹血糖有显著相关性。结论:LP-PLA2可作为糖尿病独立危险因素之一,对预测糖尿病血管并发症发生有一定价值。Objective To explore the correlation between levels of lipoprotein-associated phospholipase A2 with type 2 diabetes mellitus and its vascular complications.Methods A total of 180 cases were divided into 3 groups,type 2 diabetes mellitus complicated with coronary artery disease group（48 cases）,type 2 diabetes mellitus complicated with renal disease group（59 cases） and type 2 diabetes mellitus without vasculopathy group（73 cases）.The serum lipoprotein-associated phospholipase A2 levels were determined in these 3 groups and another 60 healthy individuals（control group）.Results The lipoprotein-associated phospholipase A2 level was significantly higher in patients with type 2 diabetes mellitus than that in control group（P0.05）,and it was higher in type 2 diabetes mellitus complicated with coronary artery disease or renal disease than that in type 2 diabetes mellitus without vasculopathy group（P0.01）.Lipoprotein-associated phospholipase A2 level was obviously correlated with low density lipoprotein,body mass index and fasting plasma glucose.Conclusion Lipoprotein-associated phospholipase A2 level is one of the independent risk factors for diabetes mellitus,and can serve as a biomarker for predicting the early vascular complications in patients with type 2 diabetes mellitus.
