结直肠癌术前MSCT检查的价值

摘　　要：目的探讨结直肠癌术前多层螺旋CT(MSCT)检查的价值。方法回顾性分析22例结直肠癌MSCT结肠成像,观察原始横断面图像,应用CT仿真结肠镜(CTVE),多平面重建(MPR),表面阴影遮盖(SSD)和透明显示(Raysum)等图像后处理技术,进行综合判断,并与术后病理及临床随访结果对照。结果 MSCT对结直肠癌定位及定性符合率为100.0%;病理分型符合率为86.4%(19/22);对DukesA期诊断敏感性100.0%,特异性94.7%;DukesB期诊断敏感性100.0%,特异性92.3%;DukesC期诊断敏感性71.4%,特异性100.0%;DukesD期诊断敏感性及特异性均为100%。结论术前MSCT检查对结肠癌分期有较高的诊断价值。Objective To investigate the diagnostic value of multi-slice spiral CT（MSCT） immaging in patients with colonic carcinomas.Methods Twenty-two patients with colonic carcinomas underwent plain MSCT colonography and multiphase conasted before operation.CTVE,MPR,SSD,and Raysum images were obtained using 4 diferent softwares in workstation.The results were compared with those of pathology and clinical follow-up.Results MSCT showed correctly the quality and location of all tumors with the accuracy rate of pathological typing of 86.4%（19/22）,the sensitivity of 100% and specificity of 94.7%.The sensitivity and specificity were of 100% and 94.7% for Dukes A,100% and 92.3% for Dukes B,71.4% and 100% for Dukes C and 100% and 100% for Dukes D.Conclusion MSCT colonography has a higher diagnostic value in staging colonic carcinoma.
