高原地区胰岛素抵抗在充血性心力衰竭中的变化及其意义

摘  要:目的观察高原地区胰岛素抵抗(IR)在充血性心力衰竭(CHF)患者中的变化及其临床意义。方法采用酶联免疫吸附法分别测定94倒CHF患者(排除糖尿病)和对照组48例同期健康志愿者的空腹血浆胰岛素(FIS)并计算空腹血糖和胰岛素乘积的倒数即胰岛素敏感性指数(IAI),以评估心衰患者的严重程度并分析各参数之间的相关性。结果心衰组的IAI低于健康组(-2.03±0.26vs-0.96±0.55,P〈0.05),并随着心功能的恶化而进一步降低(-2.04±0.22vs-2.05±0.28,P〈0.05);但慢性心衰组按病因重新分组后各组IAI相比无统计学意义(P〉0.05)。CHF组的FIS水平明显高于对照纽,差异有统计学意义(P〈0.05),随着心力衰竭程度的加重,FIS均呈进行性增高,差异有统计学意义(P〈0.05)。结论慢性心衰患者存在胰岛素抵抗(1R),且与原发病因无关,因此血清IAI水平可以作为评价高原心衰严重程度的参考指标之一。Objective To observe the changes of the IR in serum of the patients with CHF and to find new proper parameters for the monitoring of cardiac function. Methods Respectively, fasting serum insulin were measured in heart failure(CHF) group(94 cases)and control group (48 cases) assessed the degree of IR by calculating group IAI( IA1 = 1/FPG x FIS)and assayed the relations between these indices. Results Compared to the heallhy controls, patients with CHF showed significantly higher levels of FIS ( P 〈 0.05 ) and lower IAI ( - 2.03 ± 0. 26 vs -0.96 ± 0.55, P 〈 0.05) , it was also showed that the more severe the heart failure and lower the IAI. According to linear correlation regression analysis,but no correlation with the etiology of CHF was found. Conclusion IR exists in CHF patients IAI correlated each other; Therefore the levels of IAI maybe used as referential Markers in assessing the degree of CHF.
