非酒精性脂肪肝患者肝酶与C反应蛋白、胰岛素抵抗关系的研究

摘　　要：目的探讨非酒精性脂肪肝患者肝酶的变化与其胰岛素抵抗、C反应蛋白的变化的关系。方法 2007年9月至2009年9月收治的非酒精性脂肪肝病人,肝酶异常的56例,肝酶正常的62例,两组性别、年龄、体重、腰围、臀围比较无差异。禁食10h后,于第2天晨起抽取静脉血,测肝功能、空腹血糖、空腹胰岛素、C反应蛋白。结果肝酶异常组的FBG、FINS、CRP及胰岛素抵抗指数与肝酶正常组比较均有显著差异,非酒精性脂肪肝肝酶的异常与FBG、FINS、CRP及胰岛素抵抗指数的变化有关。结论肝酶异常与FBG、FINS、HOMA-IR的变化有关,另外非酒精性脂肪肝肝酶异常与CRP升高也有着密切关系。非酒精性脂肪肝一旦出现肝酶升高,同样提示肝脏有进展的炎症病变,尽早有效治疗可以防止肝脏纤维化、肝硬化的发生。Objective To explore the relationships between the changes of liver enzymes and changes of insulin resistance,C-reactive protein in patients with non-alcoholic fatty liver.Methods 118 patients with non-alcoholic fatty liver during Sep,2007 to Sep,2009 were treated,56 with abnormal liver enzymes,62 with normal.There was no difference among the gender,age,weight,waist circumference,hip circumference in the two groups.After fasting 10 hours,in the early morning of next day the liver function,fasting glucose, fasting insulin and C-reactive protein were detected.Results The results showed that the levels of FBG,FINS,CRP and insulin resistance index were different.Conclusion We should give more concern for the patients with NAFL and abnormal liver enzymes.The treatment of NAFL can improve the statuses of liver and prevent the fibrosis and cirrhosis of the liver.
