摘  要:目的对比分析血清肿瘤相关物质(Bo Xin serum tumor associated material,BXTM)在不同人群中的含量差异,探讨BXTM在肿瘤筛查、鉴别以及诊断中的临床应用价值。方法采用化学比色法检测70例恶性肿瘤患者、42例良性病变患者、28例肿瘤术后患者以及40例体检自愿者BXTM的血清水平,比较各组的BXTM水平的差异。同时测定30例原发性肝癌的AFP的浓度,24例结肠癌的CEA的浓度,比较它们与BXTM水平的相关性。结果 70例恶性肿瘤包括肺癌、肝癌、结直肠癌、乳腺癌、胃癌、食道癌、卵巢癌、血液系统肿瘤、泌尿系统肿瘤、神经系统肿瘤,恶性肿瘤组BTXM均值为(109.56±21.47)U/ml,敏感率为82.8%;良性病变组BXTM的均值为(96.35±20.82)U/ml,阳性检出率为42.8%;体检自愿者BXTM的均值为(81.16±10.03)U/ml,阳性检出率为10.0%,特异性为90.0%。恶性肿瘤患者的BXTM的浓度以及阳性率均明显高于良性病变和体检自愿者的浓度(P<0.05)。结论血清BXTM检测具有较高的敏感性、特异性和广谱性,临床上可用作恶性肿瘤筛查、鉴别以及诊断的参考指标。Objective To analyze the level of Bo Xin serum tumor associated material(BXTM) in the sera of malignant tumor patients,and to explore its clinical value in screening and differential diagnosis of malignant tumor. Methods With the method of chemical colorimetry,the level of BXTM was determined and compared in 70 cases with malignant tumor,42 cases with benign tumor,28 cases with tumor operation and 40 physical examination volunteers.Simultaneously,the level of AFP in 30 cases with primary liver cancer and the level of CEA in 24 cases with colon cancer were detected,and the correlation between AFP,CEA and BXTM was analyzed. Results Seventy patients with malignant tumor included lung carcinoma,liver carcinoma,colorectal carcinoma,breast cancer,gastric cancer,esophagus cancer, ovarian carcinoma,hematological tumor,urologic neoplasm,and nervous system neoplasm.The mean value of BXTM in malignant tumor patients was(109.56±21.47) U/ml,and the sensitive rate was 82.8%.The mean value of BXTM in benign group was(96.35±20.82) U/ml,and the positive detection rate was 42.8%.The mean value of BXTM in the control group was(81.16±10.03) U/ml,the positive detection rate was 10.0%,and the specificity was 90.0%.The level of BXTM and the positive rate of BXTM in malignant tumor patients were significantly higher than those of benign group and the control group(P〈0.05). Conclusions Detecting BXTM in malignant tumor patients has a higher sensitivity,specificity,and wide spectrum.It can be used as the indicator for the clinical screening and differential diagnosis of malignant tumors.
