探讨头皮扩张早期修复头部深度电烧伤临床应用效果

摘　　要：目的探讨头皮扩张皮瓣早期修复头部深度电烧伤骨外露的手术方法及临床效果。方法对12例高压电致头部大面积深度烧伤合并颅骨坏死外露患者,采用一期扩创、头皮下植入扩张器进行头皮扩张,二期行颅骨钻孔同时利用扩张头皮瓣移位的方法对头部创面进行早期一次修复。结果全部病例头部创面早期一次修复,创面愈合良好,随诊6个月至8年,创面头发生长茂盛,瘢痕秃发区不明显。结论应用扩张头皮瓣早期修复头部大面积深度电烧伤,具有手术简单、创伤小、疗程短、费用低和效果好的优点,同时行颅骨钻孔培育肉芽有利于扩张皮瓣与基底建立血供、加速创面愈合。Objective To investigate surgical way and clinical effect of early treatment of scalp serious electrical burn with skull exposure by scalp expansion.Methods 12 patients with scalp serious electrical burn with skull exposure due to high-voltage were observed,to debride the wound and implant the expander under the scalp on the first stage,and to bore the skull and early repair the wound using the expanded skin flaps on the second stage.Results The wounds of all patients were early repaired well.The follow-up of six months to eight years showed satisfactory hair growth,scar baldness areas can not be obviously seen.Conclusions Scalp expansion is an ideal method to early repair scalp serious electrical burn with skull exposure,with simple surgery,small areas of trauma,short duration,low-cost and good effectiveness,and so on.Boring the skull can cultivate granulation to establish the blood supply between the basement and the expanded skin flaps,and accelerate wound healing.
